摘要
目的了解西非马里人群Duffy血型系统抗原分布情况,建立西非马里人群Duffy血型抗原初步资料,改变西非马里地区的用血习惯,掌握存在的用血风险,提供临床用血信息。方法随机抽取无血缘关系的马里医院门诊术前患者的全血标本5 ml(EDTA抗凝),采用卡式凝胶法检测,使用单克隆抗-Fy^a、抗-Fy^b,按照试剂说明书进行Fy^a、Fy^b抗原检测,用正常黄种人的全血EDTA抗凝标本做阳性对照。结果 114份门诊随机采样患者中,Fy(a+b+)6例,Fy(a+b-)2例,Fy(a-b+)2例,Fy(a-b-)104例。Fy^a基因频率为0.035 3,Fy^b基因频率为0.035 3,Fy基因频率为10.198 0。经Hardy-Weinberg平衡检验,观察值与期望值之间差异有统计学意义(P<0.05);去除沉默基因重新做Hardy-Weinberg平衡检验,吻合度良好,观察值与期望值之间差异无统计学意义(P>0.05)。结论西非马里黑人人群中Fy(a-b-)抗原频率高,Fy^a和Fy^b抗原为稀有抗原。本例调查研究为西非当地建立Duffy血型资料,指导临床储血和合理使用血液提供依据。
Objective To learn the distribution of Duffy blood antigen in West African population, and to construct the data base of Duffy blood group antigen in Western African population, in order to change the blood transfusion habits in Mali of the Western Africa, to master the risk of blood transfusion, and to provide clinical information of it. Methods Blood samples (5ml, EDTA anticoagulated) were randomly collected from the outpatients in Mali Hospital. The Fy^a and Fy^b antigens were tested with monocolonal antibodies by micro - column gel agglutination assay, and normal total blood samples of yellow race were conducted for positive contrast. Results In the 114 blood samples, there were 6 cases of Fy( a + b + ) genotype, 2 cases of Fy ( a + b - ), 2 cases of Fy( a - b + ), 104 cases of Fy( a - b - ). The frequency of Fy^a gene was 0. 035 3 and that of Fy^b was O. 035 3, Fy was 10. 198 0. By the Hardy- Weinberg equilibrium test, the difference between observation value and expecfation value was statistically Significant(P 〈 0.05 ). After the elimination of silent gene, the Hardy - Weinberg test was reevaluated, showing goodness of fit, and there was no statistical significance on the difference between observation value and expectation value(P 〉 0. 05 ). Conclusion The frequency of Fy( a -b - ) is high in West Africa Mali population and Fy^a and Fy^b antigens are rare. The result of this research will provide helpful information for the data base of Duffy blood group in West African population and guide rational clinical blood reserve and safe transfusion.
