超声在胆囊癌诊断中的应用

Application of Ultrasonography in Diagnosis of Gallbladder Cancers

目的对胆囊癌超声图像进行回顾性分析,探讨胆囊癌的超声声像学特征以及超声对胆囊癌的诊断价值。方法对本院2012年1月-2015年12月收治的15例胆囊癌患者的超声声像图及术后病理结果进行对比分析,评价超声对胆囊癌的诊断价值。结果术前胆囊癌超声声像图主要表现为胆囊壁局限性或弥漫性增厚、囊内单发或多发隆起样肿物,并多数合并胆囊结石;少数表现为胆囊窝内实行肿物、胆囊结石（充满型）。15例胆囊癌中,原位癌（Tis）2例,T1a期1例,T2期7例,T3期4例,T4期1例。结论超声对于中晚期胆囊癌具有重要的诊断价值,但对于早期胆囊癌的识别具有一定的局限性。

Objective To make a retrospective analysis of the ultrasonic image of gallbladder carcinoma so as to discuss the ultrasound features of gallbladder carcinoma and the effectiveness of application of ultrasonography in diagnosis of gallbladder cancers. Methods The relationship between the histology and sonographic findings was analyzed in patients subjected to surgery for gallbladder neoplasm in the hospital during January 2012 to December 2015 so as to evaluate the effectiveness of application of ultrasonography in diagnosis of gallbladder cancers. Results Pre-operative ultrasonographic features of gallbladder carcinoma mainly included localized or diffused thickening of the gallbladder wall, isolated or multiple cystic polypoid mass together with gallbladder stones in the majority of patients; lumps and gallbladder stones（full type） in gallbladder fossa in a minority of patients. Among 15 cases of gallbladder carcinoma, there were 2 cases in carcinoma in situ（Tis）, 1 case in Stage T1 a, 7 cases in Stage T2, 4 cases in Stage T3, and 1 case in Stage T4. Conclusion Ultrasonography proves its great effectiveness in assessment of the advanced gallbladder carcinoma but has limited effectiveness in early diagnosis of gallbladder cancers.
