阿奇霉素联合特布他林和布地奈德治疗小儿支原体肺炎的临床研究

Clinical study on azithromycin combined with terbutaline and budesonide in treatment of children with Mycoplasma pneumonia

目的应用阿奇霉素联合特布他林和布地奈德治疗小儿支原体肺炎,评估其临床疗效及对患儿症状或体征的改善作用,并观察其预后,为小儿支原体肺炎的临床诊治提供依据。方法选择2014年4月-2015年2月岱山县第一人民医院儿科诊治的小儿支原体肺炎患者234例,随机分为对照组和研究组,每组各117例,对照组患儿应用阿奇霉素进行治疗,研究组在对照组治疗基础上给予特布他林和布地奈德治疗。评估两组患儿临床疗效,记录两组患儿主要症状或体征消失时间,并观察患儿不良反应。结果研究组患儿总有效率为88.89%,对照组为75.21%,两组比较,差异有统计学意义(P<0.05);研究组患儿咳嗽消失、体温恢复正常、罗音消失时间及X线阴影消失时间分别为(6.33±1.08)d、(3.21±0.58)d、(6.20±1.15)d和(6.19±1.22)d,对照组为(8.09±1.14)d、(3.96±0.51)d、(7.14±1.23)d和(7.62±1.18)d,两组比较,差异有统计学意义(P<0.05)。结论应用阿奇霉素联合特布他林和布地奈德治疗小儿支原体肺炎,临床治疗效果较好,能够有效控制感染、改善临床症状及预后,缩短患儿症状或体征改善时间,具有较高的安全性。

Objective To treat Mycoplasma pneumonia children by azithromycin combined with terbutaline and budesonide, evaluate the clinical efficacy and improvement effect on symptoms or signs of children, observe the prognosis, provide a basis for clinical diagnosis and treatment of children with Mycoplasma pneumonia. Methods A total of 234 children with Mycoplasma pneumonia were selected from De- partment of Pediatrics in the hospital from April 2014 to February 2015, then they were randomly divided into control group and study group, 117 children in each group. The children in control group were treated by azithromycin, while the children in study group were treated by az- ithromycin combined with terbutaline and budesonide. The clinical efficacies in the two groups were evaluated, the disappearance time of main symptoms or signs were recorded, the adverse reactions were observed. Results The total effective rates in study group and control group were 88. 89% and 75. 21%, respectively, there was statistically significant difference between the two groups （P〈O. 05 ） ; the disap- pearance time of cough, the time for body temperature return to normal, the disappearance time of rales and X-ray shadow were （6. 33 ± 1.08） days, （3.21±0.58） days, （6.20±1.15） days, （6. 19±1.22） days in study group and （8.09±1.14） days, （3.96±0.51） days, （7. 14± 1.23 ） days, （7.62± 1.18） days in control group, there were statistically significant differences between the two groups （P〈0.05）. Conclusion The clinical efficacy of azithromycin combined with terbutaline and hudesonide in treatment of children with Mycoplasma pneu- monia is good, the method can effectively control infection, improve clinical symptoms and prognosis, and shorten the improvement time of symptoms or signs with high sefety.
