特布他林联合布地奈德雾化吸入治疗小儿感染后闭塞性细支气管炎临床观察

Terbutaline Combined with Budesonide Aerosol Inhalation in Children with Occlusive Bronchiolitis after Infection

目的:观察特布他林联合布地奈德雾化吸入对感染后闭塞性细支气管炎患儿临床症状、炎症因子水平的影响及药物不良反应。方法:选取我院2011年5月至2015年5月收治的感染后闭塞性细支气管炎患儿110例,采用随机数字表法分为单独用药组和联合用药组各55例。两组患儿均给予常规对症治疗,包括吸氧、抗感染、肺部理疗等。单独用药组给予布地奈德雾化吸入治疗,联合用药组给予特布他林联合布地奈德雾化吸入治疗,比较两组患儿临床症状消失时间、血清IL-4及IFN-γ水平变化和药物不良反应发生情况。结果:联合用药组咳嗽、喘憋、气促、肺部啰音消失时间均短于单独用药组(P均<0.05);治疗后,联合用药组血清IL-4水平高于单独用药组,血清IFN-γ水平低于单独用药组(P均<0.05);两组患儿均治愈出院,在治疗期间均未出现明显不良反应。结论:特布他林联合布地奈德雾化吸入治疗小儿感染后闭塞性细支气管炎,可快速缓解临床症状,调节血清IL-4、IFN-γ水平,无明显不良反应,安全有效,值得临床推广应用。

Objective： To observe the clinical symptoms,levels of inflammatory cytokines and adverse reaction of terbutaline combined with budesonide aerosol inhalation in treatment of occlusive bronchiolitis after pediatric. Methods： One hundred and ten cases of occlusive bronchiolitis after infection in our hospital from May 2011 to May 2015 were selected,all children were divided into treatment group and combination group,55 cases in each group. The treatment group was given budesonide,the combination group was given terbutaline joint budesonide aerosol inhalation. The disappearance time of symptoms,serum levels of IL-4 and IFN-γ and adverse reaction were compared. Results： The disappearance time of cough,asthma,shortness of breath and rale in combination group were significantly shortened than treatment group（ P〈0. 05）. The serum level of IL-4 in combination group was higher than treatment group,the serum level of IFN-γ in combination group was lower than treatment group（ P〈0. 05）. There was no medicine-related adverse effect in both groups. Conclusion： Terbutaline combined with budesonide aerosol inhalation in treatment of occlusive bronchiolitis after infection is safe and feasible,which is worth to be spread.
