糖尿病老年患者的用药依从性综合干预效果评价

Evaluation of the effect of comprehensive intervention on medication compliance of elderly patients with diabetes

目的评价综合干预对糖尿病老年患者用药依从性的影响。方法根据就诊顺序将本院收治的200例确诊2型糖尿病的老年患者分为干预组和常规组,每组100例。两组均给予常规用药指导,此外,干预组给予健康教育、行为干预、医患沟通、患者参与、家属沟通等方式进行综合干预。分别比较两组患者住院期间、出院后随访6个月的用药依从性、血糖控制情况。结果住院期间干预组患者的用药总依从率略高于常规组(100%vs.97%),但差异无统计学意义(P>0.05)。出院后随访期间,干预组有4例患者失访,常规组有2例患者失访,干预组患者的用药总依从率显著高于常规组(98.96%vs.90.82%),差异有统计学意义(P<0.05)。随访结束时,两组患者的空腹血糖、餐后2 h血糖、糖化血红蛋白值降低(P<0.05),且干预组上述指标的控制效果优于常规组(P<0.05)。干预组的用药偏差发生率低于常规组(6.25%vs.15.31%,P<0.05)。结论综合干预对提高糖尿病老年患者用药依从性、血糖控制效果具有显著意义。

Objective To evaluate the effect of comprehensive intervention on the medication compliance of elderly patients with diabetes.Methods Totally 200 patients with type 2 diabetes were divided into intervention group and conventional group according to the order of treatment,100 cases in each group.All the patients were given routine medication guidance,and patients in intervention group received comprehensive intervention which included health education,behavior intervention,patient communication,patient participation,family communication,etc.The medication compliance and blood glucose control between the two groups were compared during hospitalization and at 6 months of follow-up.Results The total compliance rate in intervention group was slightly higher than that of conventional group（100%vs.97%） during hospitalization,there being no significant difference（P〉0.05）.There were 4 and 2 patients lost to follow-up in intervention group and conventional group during follow-up,and the total compliance rate of intervention group was higher than that of conventional group（98.96%vs.90.82%,P〈0.05）.The fasting blood glucose,postprandial 2 h blood glucose and glycosylated hemoglobin levels of patients in both groups decreased at the end of the follow-up（P〈0.05）,and the indexes in intervention group were lower than those of conventional group（P〈0.05）.The medication error incidence in intervention group was lower than that of conventional group（6.25%vs.15.31%,P〈0.05）.Conclusion Comprehensive intervention has significance in improving the medication compliance and the glycemic control of elderly patients with diabetes
