电视胸腔镜下行食管癌手术胸部淋巴结清扫的临床探讨

Video-assisted Thoracoscopic Esophageal Cancer Surgery with Chest Lymph Node Dissection

目的对电视胸腔镜下行食管癌手术胸部淋巴结清扫的临床效果进行探讨。方法方便选取该院2011年10月—2013年10月接收的120例食管癌患者作为研究对象,采用数字表法随机抽取60例进行电视胸腔镜下胸部淋巴结清扫手术,设为观察组,另外60例进行常规开胸手术,设为对照组;观察两组患者胸部各部位淋巴结平均清扫个数。结果观察组患者食管旁清扫数量(3.72±0.29)个多于对照组(3.21±1.01)个、气管旁清扫(1.92±1.36)个多于对照组(1.13±0.21)个、支气管旁、隆突下以及下肺静脉旁淋巴结的清扫数量远远多于对照组患者的清扫数量,两组比较差异有统计学意义(P<0.05)。结论食管癌患者采用电视胸腔镜手术胸部淋巴结的清扫手术情况良好。

Objective To explore the clinical effect of video-assisted thoracoscopic esophageal cancer surgery with chest lymph node dissection. Methods 120 cases with esophageal cancer admitted in our hospital from October 2011 to October 2013 were selected as the subjects and divided into the observation group and the control group in accordance with the ran- dom＂ digital table method with 60 cases in each. Patients in the observation group were treated by video-assisted thoraco- scopic esophageal cancer surgery with chest lymph node dissection, and those in the control group were treated by the con- ventional thoracotomy for esophageal cancer with chest lymph node dissection. And the average number of dissected lymph node in various parts of the chest of the two groups was observed. Results The number of dissected paraesophageal lymph node in the observation group was more than that in the control group [（3.72±0.29） vs （3.21±1.01）] （P〈0.05）. The number of dissected paratracheal lymph node in the observation group was more than that in the control group [（1.92±1.36） vs （1.13±0.21）]（P〈0.05）. The number of dissected paratracheal, subcarinal, inferior pulmonary vein lymph node in the observation group was much more than that in the control group （P〈0.05）. Conclusion Video-assisted thoracoscopic esophageal cancer surgery with chest lymph node dissection has good surgical result.
