全程中医综合干预预防孕妇产后乳房肿胀及泌乳量减少的临床观察

Clinical Observation on Whole-course Traditional Chinese Medicine Comprehensive Intervention in the Prevention of Postpartum Breast Swelling and Decrease of Milk Yield in Pregnant Women

目的：观察全程中医综合干预对孕妇产后乳房肿胀及泌乳量减少的作用。方法：选择我院2013年6月~2015年6月收治125例产妇作为研究对象,随机数字表法分为观察组（n=63）与对照组（n=62）。对照组应用产后康复仪,观察组在对照组基础上采取全程中医综合干预,比较两组护理后乳房肿胀率、泌乳始动时间及不同时间点的泌乳量。结果：观察组24h、48h、72h时的乳房肿胀率均低于对照组,差异有统计学意义（P〈0.05）。观察组泌乳始动时间在24h内的患者所占比例为69.8%（44/63）,明显高于对照组48.4%（30/62）,差异有统计学意义（P〈0.05）。观察组24h、48h、72h时的泌乳量均高于对照组,差异有统计学意义（P〈0.05）。结论：全程中医综合干预在孕妇产后中的应用利于乳房肿胀的缓解、提前泌乳时间并增加泌乳量,效果显著,值得借鉴与推广。

Objective： To observe the effect of whole-course TCM comprehensive intervention on breast swelling and decrease of milk yield in pregnant women. Methods： 125 cases of pregnant women treated in our hospital from June 2013 to June2015 were taken as the research objects and were randomly divided into observation group（ n = 63） and controlled group（ n = 62）by random number table method. The controlled group used the postpartum rehabilitation instrument. On the basis of the controlled group,the observation group was treated by whole-course TCM comprehensive intervention. After nursing,the breast swelling rates,initial time of lactation and milk yield at different time points were compared between the two groups. Results：The 24 h,48h and 72 h breast swelling rates in the observation group were lower than those in the controlled group（ P〈0. 05）.The proportion of patients whose initial time of lactation was in 24 h in the observation group [69. 8%（ 44 / 63） ]was significantly higher than that in the controlled group [48. 4%（ 30 / 62） ]（ P〈0. 05）. 24 h,48h and 72 h milk yield of the observation group was significantly higher than that of the controlled group（ P〈0. 05）. Conclusion： The application of whole-course TCM comprehensive intervention in pregnant women is conducive to the relief of breast swelling,early initial time of lactation and increased milk yield. The effect is remarkable and it is worth learning and promotion.
