卒中后认知障碍早期康复治疗的研究

Research on Early Rehabilitative Intervention for Cognitive Impairment after Stroke

目的研究卒中后认知障碍早期康复治疗对患者康复效果的影响。方法整群选取2013年1—12月该院收治的88例确诊并开始治疗的脑卒中患者,分为观察和对照两组,每组44例。对照组患者进行规范、系统的康复治疗与指导,观察组在对照组治疗的基础上再进行专门认知康复训练,并使用简易智能状态检查(mini-mental state examination,MMSE)量表、运动功能和ADL能力评分:分别采用Fugl-Meyer分级及改良Barthel指数评分对患者进行效果比较。结果康复治疗3个月后观察组患者的MMSE评分为(25.9±1.71)分、Fugl-Meyer评分(56.92±1.86)分、Barthel评分(68.13±3.64)分;对照组患者的MMSE评分为(21.68±1.7)分、Fugl-Meyer评分(41.22±1.47)分、Barthel评分(51.56±1.97)分,差异有统计学意义(P<0.05),观察组患者康复效果优于对照组。结论早期康复治疗对脑卒中患者的认知功能障碍有明显的改善作用。值得临床应用。

Objective To research the effect of early rehabilitative intervention for cognitive impairment after stroke on rehabilitative effect of patients. Methods 88 cases of patients with stroke diagnosed and treated from January 2013 to December 2013 were selected and divided into two groups with 44 cases in each, the control group were given normal and systematic rehabilitative intervention and guidance, the observation group were given special cognitive rehabilitation training, and the scales, motor functions and ADL scores were examined by mini-mental state, and the effects were compared respectively by Fugl-Meyer classification and modified Barthel index score. Results After 3 months of rehabilitative intervention, the differences in the MMSE score, Fugl-Meyer score and Barthel score between the observation group and the control group were obvious by comparison with statistical significance, [（25.9 ±1.71）,（56.92 ±1.86）,（68.13 ±3.64） vs（21.68 ±1.7）,（41.22 ±1.47）,（51.56±1.97）],（P〈0.05）, and the rehabilitative effect in the observation group was better than that in the control group.Conclusion Early rehabilitative intervention obviously improves the cognitive impairment of patients with stroke, which is worth clinical application.
