改良Miccoli技术在分化型甲状腺癌中央区淋巴结清扫的探讨

Study on Modified Miccoli Technique in Central Zone Lymph Node Dissection in Differentiated Thyroid Carcinoma

目的探究使用改良Miccoli技术在分化型甲状腺癌中央区淋巴结清扫的效果。方法整群选择2011年2月—2015年2月来该院就诊的201例分化型甲状腺癌患者为研究对象,将其分成两组,观察组实施改良Miccoli甲状腺腺叶切除+中央区淋巴结清扫术,对照组实施传统甲状腺癌标准根治术,观察患者肿瘤大小,住院时长和不良反应发生情况。结果观察组患者的肿瘤大小为(1.2±0.3)mm,住院时长为(2.5±0.3)d。手术时长为(26.12±6.35)min。术后引流量为(115.62±16.35)ml。对照组患者为肿瘤大小为(1.5±0.2)mm,住院时长为(2.9±0.5)d。手术时长为(16.43±4.85)min。术后引流量为(142.36±19.25)ml。结论患者接受改良Miccoli式中央区淋巴结清扫术,手术时间略有增加,但是可以认为安全有效,创口美观,患者更容易接受。

Objective To study the effect of modified Miccoli technique in central zone lymph node dissection in differenti- ated thyroid carcinoma. Methods 201 cases of patients with differentiated thyroid carcinoma diagnosed in our hospital from February 2011 to February 2015 were selected as the research objects and divided into two groups, the observation group implemented modified Miccoli thyroid gland lobeetomy plus central zone lymph node dissection, the control group imple- mented traditional standard radical resection of thyroid carcinoma, and the tumor sizes, lengths of stay and occurrence of adverse reactions of patients were observed. Results The tumor size, length of stay, operation time and postoperative drainage volume were respectively （1.2±0.3） mm, （2.5±0.3） d, （26.12±6.35） rain, （115.62±16.35） ml in the observation group and （1.5±0.2） mm, （2.9±0.5） d, （16.43±4.85） min, （142.36±19.25） ml in the control group. Conclusion After receiving central zone lymph node dissection of modified Miccoli-type, the operation time slightly increased, but it is still safe and effec- tive, and the trauma is beautiful, and it is more easily to be accepted by patients.
