多层螺旋CT在不典型肺结核诊断中的临床应用价值

Clinical Application Value of Multi-slice Spiral CT in Diagnosis of Atypical Pulmonary Tuberculosis

目的分析不典型肺结核采用多层螺旋CT进行诊断的临床结果。方法整群选取该院2013年6月—2015年10月收治的不典型肺结核患者34例,均采用64排128层螺旋CT扫描仪进行检查,观察诊断结果。结果 CT诊断检出28例(82.4%),和病理诊断差异有统计学意义(P<0.05)。其中发生肺部实变5例(17.9%)、结节或肿块10例(35.7%)、空洞型6例(21.4%)、阻塞性肺炎2例(7.1%)、纵膈淋巴结结核5例(17.9%)。结论不典型肺结核影像学表现具有多样化,多层螺旋CT诊断能够明确病灶类型,但检出率低,需要结合其他诊断方法。

Objective To analyze the clinical results of multi-slice spiral CT in diagnosis of atypical pulmonary tuberculo- sis. Methods 34 cases of patients with atypical pulmonary tuberculosis admitted and treated in our hospital from June 2013 to October 2015 were selected and examined by 64-row with 128-slice spiral CT scanner, and the diagnostic results were observed： Results 28 cases were diagnosed by CT （82.4%）, the difference was obvious compared with the pathological diag- nosis with statistical significance（P〈0.05）, pulmonary consolidation occurred to 5 cases （17.9%）, nodule or mass occurred to 10 cases （5.7%）, cavitary type occurred to 6 cases （21.4%）, obstructive pneumonia occurred to 2 cases （7.1%） and mediasti- hal lymphatic tuberculosis occurred to 5 cases （17.9%）. Conclusion The imaging manifestation of atypical pulmonary tuber- culosis is multiple, and the multi-slice spiral CT diagnosis can make the lesion type clear but the detection rate is low, and the other diagnostic methods need to be combined with.
