脊柱胸腰段骨折行后路椎弓根螺钉内固定联合植骨手术的临床效果

Clinical Effect of Posterior Lumbar Interbody Fusion and Bone Grafting Operation in Treatment of Thoracolumbar Fracture

目的研究分析脊柱胸腰段骨折行后路椎弓根螺钉内固定联合植骨手术的临床效果。方法整群选取并回顾分析该院2013年7月—2015年12月期间收治的脊柱胸腰段骨折患者71例,根据患者治疗方式的不同而分为观察、对照两组,观察组36例,采取行后路椎弓根螺钉内固定联合植骨手术进行治疗,对照组35例,采用联合外侧植骨方式治疗,术后随访观察3~6个月,对比分析两组患者临床疗效及相关并发症率。结果观察组患者治疗后Worter指数达到0.51±0.28,Cobb角（9.27±4.27）°,前缘高度（90.12±10.74）%,对照组Worter指数达到（0.87±0.26）,Cobb角（17.38±4.36）°,前缘高度（75.34±11.26）%,两组对比差异有统计学意义（t=2.2915,P=0.0279）;两组患者对比术后并发症,差异无统计学意义（χ~2=2.1626,P=0.1414）。结论脊柱胸腰段骨折行后路椎弓根螺钉内固定联合植骨手术疗效确切,安全可靠,值得临床推广应用。

Objective To research and analyze the clinical effect of posterior lumbar interbody fusion and bone grafting operation in treatment of thoracolumbar fracture. Methods 71 cases of patients with thoracolumbar fracture admitted and treated in our hospital from July 2013 to December 2015 were retrospectively analyzed and divided into two groups according to the different treatment methods, the observation group（36 cases） were treated with posterior lumbar interbody fusion and bone grafting operation, the control group（35 cases） were treated with posterior lumbar interbody fusion and lateral grafting,and the clinical curative effects and relevant complication rates of the two groups were compared and analyzed after 3-6month follow-up. Results The differences in the Worter index, Cobb angle and anterior height between the observation group and the control group were obvious with statistical significance（t=2.2915,P=0.0279）,[（0.51±0.28）,（9.27±4.27）°,（90.12±10.74）% vs（0.87±0.26）,（17.38±4.36）°,（75.34±11.26）%]. Two groups of patients compared the postoperative complications,there was no statistically significant（χ~2=2.1626, P=0.1414）. Conclusion The curative effect of posterior lumbar interbody fusion and bone grafting operation in treatment of thoracolumbar fracture is definite, and it is safe and reliable, which is worth clinical promotion and application.
