摘要
目的对血清心肌酶及胱抑素C水平对急性心肌梗死的临床诊断价值进行研究。方法对该院2012年1月—2015年11月诊治的急性心肌梗死患者110例进行研究,分析患者血清中心肌酶、胱抑素C及脑钠肽浓度变化与急性心肌梗死的相关性。以该院同期健康体检者110例为对照组。结果急性心肌梗死患者血清中心肌酶乳酸脱氢酶(ST段抬高型:247.2±28.7;非ST段抬高型:354.0±27.4)、磷酸肌酸激酶(ST段抬高型:149.0±13.7;非ST段抬高型:188.2±18.6)、磷酸肌酸激酶同工酶(ST段抬高型:26.7±5.31;非ST段抬高型:37.7±6.22)和脑钠素(ST段抬高型:1.82±0.19;非ST段抬高型:2.44±0.31)水平明显升高(P<0.05),而胱抑素C(ST段抬高型:1.05±0.34;非ST段抬高型:0.84±0.13)水平明显降低(P<0.05),且非ST段抬高型和抬高型患者之间差异无统计学意义(P<0.05)。结论急性心肌梗死患者存在明显的心肌酶及脑钠肽水平异常,且可作为心肌梗死类型的判断的标志物。
Objective To research the value of serum myocardial enzyme and cystatin C level in clinical diagnosis of patients with acute myocardial infarction. Methods 110 cases of patients with acute myocardial infarction diagnosed and treated in our hospital from January 2012 to November 2015 were selected for research, and the correlation between the serum myocardial enzyme, cystatin C, concentration changes of brain natriuretic peptide and acute myocardial infarction of patients was analyzed, and 110 cases of healthy examined persons in our hospital at the same period were selected as the control group. Results In the patients with acute myocardial infarction, the serum myocardial enzyme lactic dehydrogenase(ST segment elevation: 247.2±28.7; Non-ST segment elevation: 354.0±27.4), creatine phosphokinase(ST segment elevation: 149.0±13.7; Non-ST segment elevation: 188.2 ±18.6), creatine kinase isoenzyme(ST segment elevation: 26.7 ±5.31; Non-ST segment elevation: 37.7 ±6.22) and brain natriuretic peptide(ST segment elevation: 1.82 ±0.19; Non-ST segment elevation:2.44±0.31) levels obviously increased(P〈0.05), but the cystatin C level(ST segment elevation: 1.05±0.34; Non-ST segment elevation: 0.84±0.13) obviously decreased(P〈0.05), and there were obviously statistical differences between the ST segment elevation patients and the non-ST segment elevation patients(P〉0.05). Conclusion The patients with acute myocardial infarction have obvious abnormalities of myocardial enzyme and brain natriuretic peptide levels, which can be used as the markers for judging the myocardial infarction type.
