血清甘胆酸检测在妊娠期肝内胆汁淤积症中的应用价值

Application value of serum levels of serum bile acid in intrahepatic cholestasis of pregnancy

目的探讨血清甘胆酸（CG）检测在妊娠期肝内胆汁淤积症（ICP）中的应用价值。方法选择2014年1月～2015年10月于我院诊治并分娩的ICP患者80例,轻型46例（轻型ICP组）,重型34例（重型ICP组）及同期在我院分娩的正常孕妇60例（对照组）。采用放射免疫分析法检测血清CG水平,并对它们的关系进行分析。结果轻型ICP组和重型ICP组血清CG水平均高于对照组,差异有统计学意义（P〈0.05）;重型ICP组血清CG水平高于轻型ICP组,差异有统计学意义（P〈0.05）;随着血清CG水平不断增高,羊水污染、胎儿窘迫、早产、新生儿窒息、死产的发生率明显升高,C、B组羊水污染、胎儿窘迫、早产、新生儿窒息、死产的发生率高于A组,C组羊水污染、胎儿窘迫、早产、新生儿窒息、死产的发生率高于B组（P〈0.05）。结论血清CG检测在ICP患者围生期具有重要的监测价值。

Objective To investigate the value of serum cholylglycine（CG） in intrahepatic cholestasis of pregnancy（ICP）.Methods 80 cases of ICP patients from January 2014 to October 2015 in our hospital were choosed,they were divided into 46 mild cases（mild ICP group） and 34 serious cases（serious ICP group）,the control group was 60 health pregnancy women for the same period.All of cases were detected the CG level by radioimmunoassay.The results were monitored and analyzed.Results The CG levels in the ICP groups was significantly higher than that of the control group respectively（P〈0.05）.The CG level in the serious ICP group was significantly higher than that in the mild ICP group（P〈0.05）.The rates of meconium-stained,amniotic,fluid,fetal-distress,premature,neonatal asphyxia and stillbirth were increased significantly with increase of CG levels.The rates of meconium-stained,amniotic,fluid,fetal-distress,premature,neonatal asphyxia and stillbirth in the group C and group B was higher than that of group A respectively.The rates of meconiumstained,amniotic,fluid,fetal-distress,premature,neonatal asphyxia and stillbirth in the group C was higher than that of group B respectively（P〈0.05）.Conclusion CG is an important monitoring index of ICP patients in perinatal period.
