黑龙江地区肿瘤标志物CA72-4在胃癌检测中表达水平和诊断意义

Xpression Level of Tumor Marker CA72-4 in Gastric Cancer and its Diagnostic Significance in Heilongjiang

目的探讨血清中肿瘤标志物CA72-4表达水平在胃癌诊断中的意义,并探讨血清CA72-4表达水平与黑龙江地区胃癌患者年龄和分化程度的关系。方法胃癌患者、胃溃疡患者及健康对照各60例纳入研究对象,分布抽取空腹5 m L外周血,分离血清,采用电化学发光法测定血清CA72-4,化学发光免疫夹心法测定CA19-9及CEA表达水平,统计分析3组CA72-4,CA19-9及CEA表达情况;并分析血清CA72-4表达与胃癌不同分化程度及不同年龄的关系。结果胃癌组患者的血清中CA72-4、CA19-9和CEA检测水平明显高于胃溃疡组和对照组(P<0.05),胃溃疡组与对照组相比差异无统计学意义;根据胃癌组及非胃癌组(胃溃疡组、对照组)中CA72-4、CA19-9、CEA的阳性数和阴性数比较,CA72-4在胃癌诊断中的灵敏度、特异性及符合率等指标均优于CA19-9、CEA;CA72-4的表达与胃癌的分化程度密切相关,以高、中、低分化顺序依次增高;CA72-4的表达与胃癌发病年龄无关。结论 CA72-4在胃癌诊断中具有重要的意义,其对原发性胃癌的诊断较CEA和CA19-9更有价值。

Objective To explore the significance of serum tumor marker CA72-4 expression in the diagnosis of gastric cancer, and to investigate the relationship between serum CA72-4 level and the age and the degree of differentiation of gas-tric cancer patients in Heilongjiang region. Methods Patients with gastric cancer, gastric ulcer patients and health control 60 cases into the object of study, distribution of fasting 5ml peripheral blood and the serum was separated, the electrochemi-cal luminescence method for the determination of serum CA72-4, chemiluminescence immune assay determination of CA19-9 and CEA expression level, statistical analysis three groups of CA72-4, CA19-9 and CEA expression; and analysis of serum CA72-4 expression in gastric carcinoma with different degree of differentiation and relation of different ages. Re-sults Gastric cancer patients serum CA72-4, CA19-9 and CEA level was significantly higher than that of gastric ulcer group and the control group （P〈 0.05）, gastric ulcer group and the control group compared showed no statistical difference. According to the group of gastric cancer and non cancer group （gastric ulcer group and control group） of CA72-4, CA19-9 and CEA in the number of positive and negative number compared CA72-4 in gastric cancer diagnosis sensitivity, specifici-ty and coincidence rate of indicators are superior to CA19-9 and CEA, CA72-4 expression in gastric carcinoma and closely related to the degree of differentiation, high, and low differentiation order increased; CA72-4 expression and gastric cancer age has nothing to do. Conclusion CA72-4 is of great significance in the diagnosis of gastric cancer, and it is more valu-able in the diagnosis of primary gastric cancer than CEA and CA19-9.
