摘要
目的分析双向转诊机制在慢性病管理中的运用价值。方法利用医院—乡镇卫生院卫生信息平台,由医院与乡镇卫生院医生组成慢性病管理小组,形成以院前、院中、院后医疗服务为核心的双向转诊机制,并将其运用于乡镇卫生院慢性病管理中,进而分析双向转诊机制的运用效果。结果农村居民对双向转诊的知晓率有所提升;乡镇卫生院对慢性病患者采取专人分类管理,且定期为患者进行健康体检;慢性病患者得以快速有效地进行双向转诊。结论双向转诊机制的制定与实施,可促进区域卫生资源的合理运用,进而提升乡镇卫生院慢性病管理的质量。
Objective To analyze the value of Two-way Referral Mechanism in the management of chronic diseases. Methods Using the regional health information platform, by community hospitals and doctors to establish the chronic disease management team formed in pre hospital, the hospital, the hospital medical service for the core of the two-way referral mechanism, and applied to management of community chronic disease and analysis of two-way referral mechanism using the results. Results Community residents of two-way referral of awareness rate increase; community of patients with chronic diseases take special classification management and regular health examination for patients, patients with chronic diseases can be quickly and effectively for two-way referral. Conclusion The establishment and implementation of two-way referral mechanism can promote the rational use of regional health resources, and improve the quality of community chronic disease management.
