社区老年慢性疼痛患者社会支持、自我效能与生活质量的关系研究

Study on the Relationship between Social Support,Community Elderly Patients with Chronic Pain Self-efficacy and Quality of Life

目的通过调查社区老年慢性疼痛患者的人口学和一般健康相关资料、疼痛特征、社会支持、自我效能以及生活质量水平,分析人口学和一般健康相关资料、疼痛特征对社会支持、自我效能和生活质量的影响,以及社会支持、自我效能与生活质量的相关性;明确社区老年慢性疼痛患者社会支持、自我效能对生活质量的影响路径,为优化生活质量提供参考依据。方法该研究为横断面研究。便利抽取吉林市某社区60岁以上老年慢性疼痛患者220例,应用自行设计的一般资料调查问卷、社会支持评定量表、慢性疼痛自我效能量表以及简明健康状况调查量表进行问卷调查。结果社区老年慢性疼痛患者的生活质量得分为(55.29±16.90)分,其中生理健康指标得分为(48.24±16.65)分,心理健康指标得分为(62.34±19.82)分。量表8个维度(生理机能、生理职能、躯体疼痛、一般健康状况、活力、社会功能、情感职能、精神健康)得分均低于常模(t=19.031,P<0.001;t=22.878,P<0.001;t=28.594,P<0.001;t=13.021,P<0.001;t=7.433,P<0.001;t=9.793,P<0.001;t=8.548,P<0.001;t=6.233,P<0.001)。结论多元逐步回归分析结果显示:疼痛影响程度、日常收支状况、有无慢性病是社区老年慢性疼痛患者生活质量的影响因素,可解释生活质量总变异量的53.0%。

Objective To investigate the demography of community elderly patients with chronic pain and general health related data,pain characteristics,social support,self-efficacy and quality of life,analysis of the influence of demographic and general health related data,pain characteristics of social support,self-efficacy and quality of life,and the relationship between social support,self efficacy and life the quality of the patients with chronic pain;clear social elderly support,path self-efficacy impact on quality of life,provide the reference for optimizing the quality of life.Methods This study is a cross-sectional study.A convenience sample of a community of Jilin over the age of 60 elderly patients with chronic pain in220 cases,using a self-designed general information questionnaire,social support scale,chronic pain self-efficacy scale and questionnaire,concise health questionnaire.Results the scores of quality of life of elderly patients with chronic pain was（55.29±16.90）,the score of physical health index is（48.24±16.65）,the mental health index score was（62.34±19.82）.The scale of 8 dimensions（physiological function,physiological function,body pain,general health,vitality,social function,emotional function and mental health） scores were lower than the norm（t=19.031,P〈0.001;t=22.878,P〈0.001;t=28.594,P〈0.001;t=13.021,P〈0.001;t=7.433,P〈0.001;t=9.793,P〈0.001;t=8.548,P〈0.001;t=6.233,P〈0.001）.Conclusion Multiple stepwise regression analysis showed that the influence degree of pain and daily balance,having chronic diseases were the factors influencing community quality of life in elderly patients with chronic pain,quality of life can be explained 53% of the total variance.
