手术治疗退变性脊柱侧弯的疗效观察

The curative effect of surgical treatment of degenerative scoliosis

目的观察手术治疗退变性脊柱侧弯的临床效果。方法 90例退变性脊柱侧弯患者按照完全随机化原则分为观察组和对照组各45例,对照组:对患者发生神经和脊髓压迫的部位进行减压处理,进行植骨和复位治疗。观察组:对患者进行充分的减压处理,采用长节段固定以及短节段融合的方法进行治疗,并予以脊柱重建,使得冠状面和矢状面得到平衡。结果观察组患者的植骨全部融合,为100%,平均融合时间为(4.1±0.9)个月,手术之后的Cobb角为(11.3+2.5)°;对照组中30例患者植骨完全融合,占总数的66.7%,平均融合时间为(6.7±1.5)个月,手术之后的Cobb角为(19.3+2.7)°。观察组明显优于对照组,差异有统计学意义(P<0.05)。结论长节段固定以及短节段融合治疗脊柱侧弯,能够有效改善患者的生活质量,值得推广应用。

Objective To observe the effect of surgical treatment of degenerative scoliosis.Methods 90 cases of degenerative scoliosis patients randomly assigned,Divided into observation group and control group.The Control group：For part of the nerve and spinal cord compression in patients with treatment,And for the treatment of patients with bone graft and reset.Observation group：For patients with adequate decompression treatment,Using short segmental fixation and long segmental fusion method for the treatment of patients with,And the patients with reconstruction of the spine,The coronal and sagittal balance.Results After treatment,carried out 12 to 24 months of follow-up observation group：patients with bone graft fusion,was 100﹪;the average fusion time was（4.1±0.9）months after the surgery Cobb angle was（11.3＋2.5）°;control group：30 patients with complete bone fusion,66.7﹪ of the total;the average fusion time was（6.7±1.5）months after the surgery（11.3＋2.5）°.The two groups were compared,the test group than the control group,the difference was statistically significant（P﹤0.05）.Conclusion In the treatment of scoliosis,can effectively improve the patient′s quality of life,is worth popularization and application.
