改良型鼻胆管口鼻转换磁石牵引管的临床应用价值 被引量:7

摘要 〔目的〕改良的鼻胆管口鼻转换高性磁铁牵引软管在提高鼻胆管口鼻转换过程中的临床应用价值分析和评价。〔方法〕将80例行鼻胆管引流术的患者随机分为磁石组和导丝组。磁石组采用自主研制的鼻胆管经鼻导出的高性磁铁软管,将装有高性磁铁的一端经口送至口咽部,另一端经鼻腔进入金属头软管,相互吸引后顺利导出鼻胆管。导丝组采用导丝U型圈从咽后壁牵引拉出插入的胃管再将鼻胆管导出。〔结果〕磁石组的用时较短,成功率高,一次性导出成功率明显高于导丝组。成功率:磁石组100%(40/40),导丝组95%(38/40),差异无统计学意义。一次成功率:磁石组97%(38/40),导丝组75%(30/40),差异有统计学意义。平均所需时间:磁石组1.1 min,导丝组2.2 min,差异有统计学意义。并发症发生率:磁石组5%(2/40),导丝组20%(8/40),差异有统计学意义。〔结论〕采用高铁磁性导管导出鼻胆管一次成功率高,简单易行,耗时较短,患者反应普遍轻,避免口腔黏膜的损伤,减少患者的痛苦,提高手术效率。 〔Objective〕Mproved nasobiliary oronasal conversion high magnet traction hose in improving nasobiliary oronasal conversion in the process of clinical application value analysis and evaluation.〔Methods〕80 cases of nasal biliary drainage were randomly divided into group A and group guide wire, A group with nasal bile duct independently developed by high plastic hose nose is derived, One end of the magnet is provided with one end of the magnet to be delivered to the throat, At the other end of the nose into the metal hose, After each other to attract the smooth export of nasal bile duct, The guide wire group with guide wire U ring insertion tube drawn out from the nasal duct derived posterior pharyngeal wall. 〔Results〕A group of short time, high success rate, The success rate was significantly higher than that of the guide wire group. Success rate: group 100%( 40 / 40), a guide wire group 95%( 38 / 40), the difference was not statistically significant. success rate of 97%( 38 / 40) : magnet group, the guide wire group 75%( 30 / 40), the difference was statistically significant. The average time required: a group of min( 1. 1 +0.9), the guide wire group 2.2( +25-53) min, the difference was statistically significant. The incidence of complications: group5%( 2 / 40), a guide wire group 20%( 8 / 40), the difference was statistically significant. 〔Conclusion〕The high speed and high efficiency of the nasal bile duct is obtained by using the high ferromagnetic catheter, which is simple and easy to operate, with a short time, and the patients are generally mild, so as to avoid the injury of the oral mucosa, reduce the pain of the patients, and improve the operation efficiency.
