远红外热像图对烟雾病诊断价值的探讨

摘要 目的通过分析烟雾病患者远红外热像图的变化,探讨红外热像仪在烟雾病诊断中的价值。方法将解放军总医院临床诊断为烟雾病的患者28例作为患者组,30例健康体检者作为正常对照组,受检者暴露检查部位。检查在环境温度为20℃~25℃的检查室中进行。使用远红外热像仪采图,同时对人体头面部的红外热像图进行分析,观察烟雾病患者组与正常对照组的身体各部位左右两侧的温差变化,并进行统计学分析。结果28例烟雾病患者的红外热像图中,头面部充血性改变呈红色或深红色,缺血性改变呈蓝色或浅绿色。患者组的额颞区、内眦区两侧对称区域温度差值明显大于正常组,差异有统计学意义(t=1.453,1.636,P〈0.05)。结论红外热像图可通过皮肤温度变化,敏感地反映烟雾病患者的身体热分布及功能状况,对于烟雾病诊断具有重要价值。 Objective To assess the value of far-infrared thermography in the diagnosis of Moyamoya disease by analyzing the alterations in the thermogram. Methods A total of 28 patients with Moyamoya disease were assigned as observation group, and another 30 healthy individuals who taking physical examination as control group. Far-infrared thermography was carried out on the exposed inspection sites under an environmental temperature from 20℃ to 25℃. The craniofacial infrared images were collected and analyzed for temperature differences among different inspection areas and the right and left sides of the examinees. The results were analyzed statistically. Results All Moyamoya disease cases showed some typical patterns in far-infrared thermograms, with congestive changes as red or deep red, and ischemic changes as blue or mild green on the head and face. Temperature differences in bilaterally symmetrical parts of the frontotemporal region and medial canthal region were significantly larger in the observation group than in the control group ( t = 1. 453, 1. 636 ; P 〈 0. 05 ). Conclusion Far-infrared thermography sensitively reflects heat distribution and functional status in Moyamoya patients through detecting skin temperature changes, and is of great significance for the diagnosis of the disease.
