软通道与硬通道微创手术治疗高血压性脑出血的临床疗效对比研究

A comparative study of clinical efficacy between soft and hard channel minimally invasive surgery in treatment of hypertensive cerebral hemorrhage

目的比较软通道与硬通道微创手术治疗高血压性脑出血的疗效差异。方法选取2012年1月至2015年3月我院90例高血压性脑出血患者作为研究对象,随机分为软通道组和硬通道组,每组各45例,软通道组采用软通道微创穿刺术治疗,硬通道组采用硬通道微创穿刺术治疗,随访6个月,综合比较两组患者格拉斯哥预后评分(GOS)及并发症发生率的差异。结果软通道组患者恢复良好率为17.8%,与硬通道组比较差异无统计学意义(P>0.05),软通道组植物生存9例,死亡2例,与硬通道组比较差异无统计学意义(P>0.05);两组患者术后出现肺部感染、消化道出血、气颅、再出血等并发症发生率比较差异无统计学意义(P>0.05)。结论软通道与硬通道微创手术治疗高血压性脑出血效果相当,各有优缺点,临床医师需根据患者的实际情况制定个体化手术方案,以降低死亡率,改善预后。

Objective To compare the difference of the curative effect of soft and hard channel minimally invasive surgery in the treatment of hypertensive cerebral hemorrhage. Methods Selected 90 cases of hypertensive cerebral hemorrhage patients as the research object admitted in our hospital from January,2012 to March,2015,who were randomly divided into soft channel group and hard channel group,each group had 45 cases,the soft channel group was given with minimally invasive puncture treatment by the soft channel,the hard channel group was given with minimally invasive puncture treatment by the hard channel,which were followed up for 6 months,compared Glasgow outcome scale（ GOS） and the incidence of complications of two groups.Results The good recovery rate of soft channel group was 17. 8%,there was no statistically significant difference compared with hard channel group（ P 0. 05）,the soft channel group 9 cases of plants survival,death in 2cases,there was no statistically significant difference compared with hard channel group（ P 0. 05）; the postoperative pulmonary infection,gastrointestinal bleeding,gas cranial and incidence complications such as bleeding of two groups were compared,the differences had no statistical significance（ P 0. 05）. Conclusions The curative effect of soft and hard channel minimally invasive surgery in the treatment of hypertensive cerebral hemorrhage is same,each group has advantages and disadvantages,the clinician should be designated individual operation scheme according to the actual situation of patients,in order to reduce the mortality and improve prognosis significantly.
