少见十二指肠间质瘤的诊治体会

Rare experience of diagnosis and treatment of the duodenal stromal tumor

[目的]探讨十二指肠间质瘤的临床特点及诊治方法。[方法]回顾性分析近13年来我院确诊的16例十二指肠间质瘤患者的临床资料。[结果]本组中女性患者占75%;≥60岁者占68.75%,<60岁者占31.25%;发病部位以十二指肠降部较多见(62.5%);呕血、黑便或便血为常见临床表现(75%)。本组患者均行手术治疗。6例术后服用甲磺酸伊马替尼400mg/d治疗。[结论]十二指肠间质瘤好发于老年人,十二指肠降部为好发部位,上消化道出血是常见临床表现。内镜和CT检查是主要的辅助检查手段。外科手术为首选治疗方法。

[Objective]To explore the experience of diagnosis and treatment of duodenal stromal tumor of the clinical characteristics.[Methods]16patients with duodenal stromal tumor were retrospectively analyzed in our hospital.[Results]More female cases（12/16,75%）in our group;11cases（68.75%）aged above 60;The tumor mainly involved of the descending duodenum（10/16,62.5%）;Haematemesis,melena,or blood in the stool was a common clinical manifestations（12/16,75%）.All patients were treated with operation and complete resection of the tumor,2cases of malignant,4cases of borderline stromal tumors,and 10 cases were benign tumor,6of the patients taking Mesylate Imatinib 400mg/d postoperatively.[Conclusion]Duodenal stromal tumor prensented more common in the elderly,the descending duodenum was the predilection site,and upper gastrointestinal hemorrhage was the common clinical manifestation.Endoscopy and CT are the main means of auxiliary examination,The surgery is the preferred method of treatment.
