急诊手术处理踝关节骨折脱位患者的临床效果

Clinical effect of emergency surgical treatment for patients with ankle fracture and dislocation

目的:探讨踝关节骨折脱位患者采用急诊手术治疗的临床效果。方法:选取我院2012-01-2015-06收治的69例69踝进行回顾性分析,其中37例患者采取急诊手术治疗(急诊组)、32例患者采取择期手术治疗(择期组),对比2组的手术效果差异。结果:急诊组和择期组的手术时间、骨折愈合时间比较差异无统计学意义,急诊组的术后住院时间、总医疗费用均显著的低于择期组患者,差异具有统计学意义(P<0.05);急诊组的临床评分、骨折复位评分、骨性关节炎评分与择期组比较,差异无统计学意义;末次随访时,急诊组的Phillips踝关节功能优良率83.78%与择期组的81.25%比较,差异无统计学意义。结论:踝关节骨折脱位患者采用急诊手术治疗的手术效果与择期手术无差异,但有利于减少患者的住院时间和治疗费用。

Objective：To investigate the clinical effect of emergency surgical treatment for patients with ankle fracture and dislocation.Method：Sixty-nine ankles of 69 cases in the hospital in January 2012～June 2015 were selected and analyzed retrospectively,in which 37 patients taking emergency surgery in emergency group,32 patients taking elective surgery in elective group.The difference in results of operations was compared between two groups.Result：Difference in the operation time,fracture healing time was not statistically significant between the two groups.Postoperative hospital stay,total medical costs in emergency group were significantly lower than that in elective group and the difference was statistically significant（P〈0.05）;There were no significant difference in clinical scores,fracture reduction rates,osteoarthritis score between two groups;at last follow-up,Phillips ankle joint function excellent rate in the emergency group was 83.78% and 81.25%in elective group,which was not statistically significant.Conclusion：There was no difference in effect of treatment for ankle fracture and dislocation using or elective surgery.But emergency surgery helps to reduce the length of stay and cost of treatment of patients.
