雷火灸预防混合痔患者术后尿潴留的效果研究

Efficacy of Thunder-fire Moxibustion for Preventing Postoperative Uroschesis of Mixed Hemorrhoids Patients

为探讨雷火灸预防混合痔术后尿潴留的效果，将120例混合痔术后患者随机分为观察组和对照组，各60例。观察组术后2h给予雷火灸治疗，取穴关元、中极，灸20min，以熏灸部位产生温热感而无灼痛且皮肤红晕为度，治疗后协助患者自行排尿；对照组术后2h鼓励患者自行排尿，当患者有明显尿意感而不能自行排尿时给予常规护理，用热毛巾湿热敷小腹部，并给予听流水声诱导排尿。对比两组患者排尿情况。结果显示，观察组术后8h内无尿潴留病例，术后24h内尿潴留1例（1．6％）。对照组术后8h内尿潴留8例（13．3％），术后24h内尿潴留9例（15．0％）。观察组术后8h内及12h内排尿情况明显优于对照组，P〈0.01。观察组首次排尿时间[（2．96±1．21）h]早于对照组E（s．48±1．38）h]，P〈0.01。结果表明，混合痔术后2h予雷火灸治疗，能有效预防尿潴留的发生，降低术后导尿率，减轻患者的痛苦，值得临床推广应用。

This study was to investigate the preventive efficacy of thunder-fire moxibustion on postoperative uroschesis of mixed hemorrhoids patients. The 120 patients subjected to mixed hemorrhoids surgery were randomly divided into observation group and control one,60 cases for each;2 h after surgery the former received thunder-fire moxibustion at Guanyuan（CV4） and Zhongji（CV3）, for 20 rain until moxibustion site patients felt warm-heat, skin became flush, but without ardor, then, were given help to urinate auto- matically^while the latter were encouraged to urinate by patients-self within 2 h after surgery,only when patients felt precipitant urination but un-automatically urinate,just were given moist-heat towel to spread their lower abdomen and made them hearing water-flowing sound so inducing urination ; then , both group＇s urination status were compared.As results,in the former within 8 hrs after surgery there was no uroschesis cases,and within 24 hrs only one case surffered from uroschesis（1.6%）, while in the latter respectively 8 cases（13.3 %）, and 9 cases （ 15.0 %）, thus the former was significantly superior to the latter （ P 〈 0.01） And, the first-time urination of the former was significantly earlier than that of the latter[（2.96± 1.21）h vs （5.48±1.38）h, P 〈0.01].Results show that at 2 h after surgery performing thunder-fire moxibustion on mixed hemorrhoids patients can effectively prevent uroschesis, reduce postoperative catheterization rate, relieve patient＇s pain,thus which is worthy to be popularized to use clinically.
