制霉菌素联合碳酸氢钠预防肿瘤患者放疗后口腔感染的分析

Clinical effect of nystatin combined with sodium bicarbonate on prevention of oral infections in tumor patients after radiotherapy

目的分析制霉菌素联合碳酸氢钠预防肿瘤患者放疗后口腔感染的临床效果,为患有口腔感染的患者寻找到最适合的治疗方法。方法选取2013年1月-2014年2月于医院接受肿瘤放疗患有口腔感染的患者300例,随机分为试验组和对照组,每组各150例,对照组患者给予生理盐水、口泰含漱液漱口,试验组患者给予制霉菌素联合碳酸氢钠含漱液漱口,调查两组患者口腔感染率及并发症发生率,分析不同治疗效果;采用SPSS 15.0软件进行统计分析。结果对照组患者口臭发生率为6.00%、试验组为1.33%,对照组牙龈肿痛发生率为6.67%、试验组为2.00%,对照组口腔溃疡发生率为14.00%、试验组为2.00%,对照组口腔真菌斑发生率为10.67%、试验组为2.67%,以上两组数据比较,差异均有统计学意义(P<0.05);在接受相关治疗后,试验组患者口腔pH值为4.8±0.3、对照组为7.5±0.6,两组数据比较差异有统计学意义(P<0.05)。结论制霉菌素联合碳酸氢钠预防肿瘤患者放疗后预防口腔感染的效果比较显著,可明显降低口腔感染以及口臭和口腔溃疡等相关病症,有效提高患者的口腔pH值。

OBJECTIVE To observe the clinical effect of nystatin combined with sodium bicarbonate on prevention of oral infections in tumor patients after radiotherapy so as to seek the most appropriate treatment method for the patients with oral infections.METHODS A total of 300 tumor patients with oral infections who received the radiotherapy in the hospital from Jan 2013 to Feb 2014 were enrolled in the study and randomly divided into the experimental group and the control group,with 150 cases in each group.The control group was treated with normal saline,mouth Thai gargles for oral gargle,while the experimental group was given nystatin combined with sodium bicarbonate for mouth gargle.The incidence rates of oral infections and complications were investigated,the curative effects were observed,and the statistical analysis was performed with the use of SPSS 15.0software.RESULTS The incidence rate of bad breath was 6.00%in the control group,1.33%in the experimental group;the incidence rate of sore gums was 6.67%in the control group,2.00%in the experimental group;the incidence rate of oral ulcers was 14.00%in the control group,2.00% in the experimental group;the incidence rate of oral plaque was10.67%in the control group,2.67%in the experimental group;as compared with the two sets of data,there was significant difference（P〈0.05）.The oral pH value was 4.8±0.3in the experimental group after the related treatment,7.5±0.6in the control group,and there was significant difference between the two sets of data（P〈0.05）.CONCLUSION Nystatin combined with sodium bicarbonate can achieve significant effect on prevention of oral infections in the tumor patients after the radiotherapy,remarkably reduce the incidence of oral infections and bad breath as well as oral ulcers,and effectively improve the oral pH value.
