“标本配穴”温针灸联合头背部推拿治疗脑梗死后失眠的疗效观察

Biao- Ben Points Association Warm- Acupuncture Combined with Head and Back Massage Therapy on Insomnia after Cerebral Infarction

目的:观察"标本配穴"温针灸联合头背部推拿治疗脑梗死后失眠的疗效,并探讨其作用机制。方法:90例脑梗死后失眠患者随机分为针推组、针灸组及西药组,每组30例。针推组给予"标本配穴"温针灸结合头背部推拿干预治疗,针灸组给予单纯"标本配穴"温针灸治疗,西药组给予口服艾司唑仑片。观察和比较3组患者治疗前、治疗结束时及治疗结束后4周时匹兹堡睡眠质量指数(PSQI),进行血液流变学检测,比较3组治疗后的近期疗效和远期疗效。结果:1与治疗前比较,治疗结束时各组PSQI评分均明显下降(P<0.05);与治疗结束时比较,治疗后4周针推组和针灸组两组睡眠情况无明显反复(P>0.05),西药组则明显反复(P<0.05)。2与治疗前比较,治疗结束时针推组和针灸组各血流变指标均明显下降(P<0.05),且针推组下降程度明显高于针灸组(P<0.05),而西药组各血流变指标变化不明显(P>0.05)。3针推组的近期和远期临床总有效率均明显优于其他两组(P<0.05)。结论:"标本配穴"温针灸联合头背部推拿能显著降低脑梗死后失眠患者的血液粘稠度,有效改善睡眠质量,具有较好的近期和远期疗效。

Objective： To observe the clinical effect of the Biao- Ben points association warm- acupuncture combined with head and back massage therapy on the treatment of insomnia after cerebral infarction. Methods： Ninety cases diagnosed as insomnia after cerebral infarction were randomly divided into acupuncture- massage group,acupuncture group and western medicine group,30 cases in each group. Acupuncture- massage group was treated with Biao- ben points association warm- acupuncture combined with head and back massage therapy,acupuncture group with pure warm- acupuncture and western medicine group with oral Estazoolam tablets. Pittsburgh sleep quality index（ PSQI） was observed before and after treatment and 4 weeks after treatment in three groups,meanwhile,blood rheology was detected. The short and long- term curative effects were compared after treatment. Results： 1 Compared with those before treatment,the scores of Pittsburgh sleep quality index were obviously decreased after treatment in all groups（ P〈0. 05）. Compared with those at the end of treatment,the repetition phenomenon was not obvious in acupuncture- massage group and acupuncture group（ P〉0. 05）,but obvious in western medicine group（ P〈0. 05）. 2Compared with those before treatment,the indexes of blood rheology were obviously decreased after treatment in acupuncture- massage group and acupuncture group（ P〈0. 05） but not obviously in western medicine group（ P〉0. 05）. 3After treatment,the short and long- term total effective rates of acupuncture- massage group were obviously higher than the other two groups＇（ P〈0. 05）. Conclusion： Biao- ben points association warm- acupuncture combined with head and back massage therapy can both significantly reduce the blood viscosity and improve sleep quality on the patients of insomnia after cerebral infarction. The comprehensive therapy has a good recent and forward curative effect.
