疤痕子宫再次妊娠经阴道分娩的可行性研究

Study the feasibility of vaginal delivery for scar uterine pregnancy again

目的探讨疤痕子宫再次妊娠经阴道分娩的可行性。方法选取2014年8月至2015年10月我院收治的疤痕子宫再次妊娠产妇110例为研究对象,将全部产妇依据分娩方式分为两组,观察组与对照组分别有55例产妇,对照组产妇实施剖宫产,观察组产妇经阴道分娩,比较两组产妇的分娩结局。结果对照组分娩成功率是100%,观察组分娩成功率是92.73%,两组母婴存活率均为100%,两组产妇的分娩成功率、母婴存活率以及分娩时间比较无显著性差异;观察组产妇的产时出血量与住院时间明显少于对照组,差异具有显著性;对照组并发症发生率是23.64%,观察组并发症发生率是3.64%,对比两组产妇的并发症发生情况,观察组明显少于对照组,具有统计学差异。结论临床为疤痕子宫再次妊娠产妇选择分娩方式时,对于符合阴道试产指征的产妇应积极采取阴道试产;疤痕子宫再次妊娠经阴道分娩具有较高的安全性与可行性,值得在临床上加以重视。

Objective To explore the feasibility of vaginal delivery for scar uterine pregnancy again.Methods During August 2014 to October 2015,110 cases of scar uterine pregnancy in our hospital were selected as the objects of this study,all women were divided into two groups according to the mode of delivery,55 cases in each group,mothers in the control group taken caesarean section,maters in the observation group chose vaginal delivery,maternal birth outcomes were compared. Results The success rate of birth in control group was 100%,the delivery success rate of observation group was 92. 73%,maternal and child survival rates of the two groups were 100%,respectively. The success rate of maternal childbirth,maternal and child survival rate and delivery time has no significant contrast difference. Intrapartum bleeding in the observation group and maternal hospital stay was significantly less than the control group,the difference was significant. The incidence of complication rate of control group was 23. 64%,which was significantly less than the control group（ 3. 64%）.Conclusions When choose the mode of delivery,in accordance with the indications of maternal vaginal delivery should take vaginal delivery. Vaginal delivery of scar uterine pregnancy again with high safety and feasibility,it is worth clinical attention.
