摘要
目的对比分析低位切除结合高位松挂线分次紧线术与低位切开高位挂线术治疗高位复杂性肛瘘的临床疗效。方法选择2012年3月至2015年3月在我院住院治疗的高位复杂性肛瘘患者86例作为研究对象,按照随机数字表法将其分为观察组和对照组,每组各43例,观察组采用低位切除结合高位松挂线分次紧线术进行治疗,对照组采用低位切开高位挂线术进行治疗,对比两组临床疗效,观察肛门括约肌功能情况。结果观察组患者总有效率为95.34%显著高于对照组患者的79.07%(P<0.05);观察组患者平均住院时间、创口愈合时间、挂线脱落时间均显著低于对照组(P<0.05);两组术后肛门括约肌功能良好率比较差异具有统计学意义(P<0.05)。结论低位切除结合高位松挂线分次紧线术对高位复杂性肛瘘的治疗效果显著,对肛门括约肌损伤较小,值得临床推广。
Objective Comparatively analysis the clinical efficacy of low resection combined with high loose seton tight thread gradually and low incision surgery with high seton in the treatment complex anal fistula.Methods From March 2012 to March 2015,86 cases of patients suffered from high complexity anal fistula and hospitalized in our hospital were selected as research subjects. They were randomly divided into observation group and control group according random number table,43 cases in each group. Observation group taken low resection combined with high loose seton frapping thread surgery for treatment,however,the control group received high thread- bit incision surgery for treatment. Compared the clinical efficacy and observed the anal sphincter function. Results The total cure rate of observation group was 95. 34% and it was 79. 07% in control group,so the observation group was significantly higher（ P〈0.05）. The average length of hospital stay,wound healing time,hung off time of patients in the observation group were significantly lower than the control group（ P〈0.05）. The difference postoperative anal sphincter function relatively good rate between the two groups was statistically significant（ P〈0.05）. Conclusions Low resection combined with combined with high loose seton tight thread gradually for the treatment of high complex anal fistulas obtain good efficacy,it has less damage to the anal sphincter,worthy of promotion.
