综合护理干预对急性脑梗死患者负性情绪及生活质量的影响观察

Influence of comprehensive nursing intervention on negative emotion and quality of life for patients with acute cerebral infarction

目的探讨综合护理干预应用于急性脑梗死患者的临床效果及对负性情绪和生活质量的影响。方法选取2013年3月至2015年8月我院90例急性脑梗死患者,随机分为两组,研究组和对照组各45例。对照组采用常规护理,研究组采用综合护理干预,采用汉密尔顿焦虑量表-14项(HAMA-14)、汉密尔顿抑郁量表-21项(HAMD-21)评价心理状态,采用SF-36量表评价生活质量,比较两组患者心理状态及生活质量评分的差异。结果综合护理干预组HAMA-14、HAMD-21评分分别为(10.4±3.2)分、(15.8±4.8)分,低于对照组,组间比较差异有统计学意义(P<0.05);研究组患者SF-36各维度评分明显优于对照组(P<0.05)。结论综合护理干预应用于急性脑梗死患者效果确切,能有效减轻患者的焦虑、抑郁情绪,减轻病残程度,提高生活质量,值得临床推广使用。

Objective To explore the clinical effect of comprehensive nursing intervention on patients with acute cerebral infarction and its effect on negative emotion and quality of life. Methods A total of 90 patients with acute cerebral infarction in our hospital from March 2013 to August were randomly divided into study group（ n = 45） and control group（ n = 45）. Patients in the control group received routine nursing care while the patients of study group received comprehensive nursing intervention. Compared the differences between patients from two groups on the mental state by Hamilton Anxiety Scale-14 items（ HAMA- 14） and Hamilton depression amount table- 21 items（ HAMD- 21） and the quality of life by the SF- 36 scale. Results The scores of HAMA- 14（ 10. 4 ± 3. 2） and HAMD- 21（ 15. 8 ± 4. 8） of study group were both lower than that of the control group（ P〈0.05）. All the dimensions of SF- 36 score of the study group were significantly better than that of the control group（ P〈0.05）. Conclusions Our results suggest that comprehensive nursing intervention is worthy of clinical use for it can exert a promotion effect on acute cerebral infarction by relieving anxiety and depression of the patients,reducing the degree of disability and improving the quality of life.
