参麦注射液治疗左室射血分数正常心衰临床研究

The Curative Effect of Shenmai Injection in Treating Heart Failure with Normal Ejection Fraction

目的:探讨参麦注射液治疗左室射血分数正常心衰(HFNEF)的疗效。方法:选取85例HFNEF患者,随机分为2组,在常规西药治疗的基础上,治疗组静脉滴注100m L参麦注射液,对照组静脉滴注100m L葡萄糖水,均为1次/d,疗程为14d。比较2组患者用药前后临床症状、血浆脑钠肽(BNP)、左室舒张末期内径(LVEDd)、舒张早期血流峰速度与舒张晚期血流峰速度比值(E/A值)、6分钟步行距离(6MWT)以及中医证候积分等指标,并对其进行临床分析。结果:对照组及治疗组治疗后与2组治疗前相比,患者临床症状显著改善(P<0.05),血浆BNP、中医证候积分显著降低(P<0.05),6MWT显著提高(P<0.05);治疗组治疗后与对照组治疗后相比,患者临床症状显著改善(P<0.05),血浆BNP、LVEDd及中医证候积分显著降低(P<0.05),E/A值、6MWT显著提高(P<0.05)。结论:常规西药治疗HFNEF可以改善患者临床症状,参麦注射液可进一步改善HFNEF患者心脏舒张功能,提高运动耐量,降低中医证候积分,从而进一步改善患者的生活质量。

Objective： To observe the curative effect of Shenmai injection in treating heart failure with normal ejection fraction （HFNEF）. Methods： 85 cases with HFNEF were randomly divided into 2 groups. The 2 groups were given routine treatment. Each patient in treatment group was intravenously injected with 100ml Shenmai injection; and 100ml glucose was injected intravenously to the patients in control group. The experiment lasted 14 days and the injection frequency was 1 time a day. The clinical symptom; brain natriuretic peptide （BNP）; left ventricular end-diastolic diameter （LVEDd）; E/A ratio; six minutes walk test （6MWT）; traditional Chinese medicine syndrome （CTMS） were compared between the two groups, Results： Compared with before treatment; the clinical symptoms; 6MWT of 2 groups after treatment were all improved （P〈0.0 5）; BNP and CTMS decreased （P〈0.05）. Compared with the control group; the clinical symptoms; E/A ratio; 6MWT of patients who were given Shenmai Injection improved significantly（P〈0.05）; BNP; LVEDd and CTMS decreased （P〈0.05）. Conclusions： Routine treatment can improve the clinical symptoms： Shenmai injection can further improve the cardiac diastolic function; increase exercise tolerance; decrease CTMS; thus it can further improve the quality of life of patients.
