鼻内镜术治疗慢性鼻-鼻窦炎患者的临床疗效评价

Clinical effect of nasal endoscopic surgery for chronic nasal sinusitis

目的探讨鼻内镜术式对慢性鼻-鼻窦炎患者的临床疗效及症状评分的影响。方法选取经耳鼻喉科收治的慢性鼻炎鼻窦炎患者124例作为研究对象。依照数字法随机分为观察组62例与对照组62例。对照组采用常规手术治疗;观察组实施鼻内镜术式治疗。比较两组患者术后半年嗅觉障碍、头痛、鼻塞、鼻分泌物等症状评分及生活质量评分。生活质量评分采取SF-36生活质量量表评估患者生活质量,其中包括8个维度;选择总体健康、生理功能、社会功能、情感职能、精神健康5个维度进行评分。评分越高,生活质量越高。结果术后半年患者嗅觉障碍、头痛、鼻塞、鼻分泌物等评分相比术前显著降低,且观察组降低程度显著高于对照组,差异均有统计学意义(P<0.05)。经治疗后,观察组总体健康、生理功能、社会功能、情感职能、精神健康评分显著提高,且提高程度明显高于对照组,差异均有统计学意义(P<0.05)。观察组疗效为优者占比67.74%,优良率为90.32%,均显著高于对照组的50.00%和69.35%。差异均有统计学意义(P<0.05)。经治疗后观察组术后复发率8.06%、并发症发生率4.84%显著低于对照组的20.97%和17.74%,差异均有统计学意义(P<0.05)。结论对慢性鼻炎鼻窦炎患者实施鼻内镜手术治疗能显著改善患者症状,提高患者生活质量,降低术后并发症的发生与复发率,疗效显著,安全性高,可在临床推广应用。

Objective To explore the curative effect of nasal endoscopic surgery for chronic nasal sinusitis.Methods 124 chronic nasal-sinusitis patients were randomly divided into observation group（62cases）and control group（62cases）.The control group was treated with conventional surgical treatment.The observation group was treated with implementation of nasal endoscopic surgery.The olfactory disorder,headache,stuffy nose,nasal discharge treatment and symptom score and quality of life scores were observed.Quality of life was evaluated with SF-36 including overall health,physical function,social function,emotional function,mental health score five dimensions.Results Six months after olfactory dysfunction,the headache,stuffy nose and nasal secretions significantly reduced after treatment.The extent of the observation group was significantly higher than that of the control group（all P0.05）.After treatment,the observation group′s overall health,physical function,social function,emotional function,mental health score increased significantly,and incensement was significantly higher than that of control group（P0.05）.The postoperative recurrence rate and incidence of complications of observation group were 8.06% and 4.06%,which were significantly lower than that of the control（20.97% and 17.74%）（P0.05）.Conclusion Implementation of nasal endoscopic surgery in patients with chronic nasal-sinusitis treatment can significantly improve the quality of life in patients and reduce the occurrence of postoperative complications and the recurrence rate.
