益气活血利水方对扩张型心肌病严重心衰的临床观察

Clinical observation on treating severe heart failure of dilated cardiomyopathy with the Yiqi Huoxue Lishui decoction

目的:本文主要观察益气活血利水方治疗扩张型心肌病严重心力衰竭患者的疗效及安全性。方法:选择扩张型心肌病心功能NYHA分级III^IV级患者45例,随机分为治疗组(23例)和对照组(22例),对照组为标准抗心衰治疗,治疗组在上述基础上加用益气利水方,疗程为2周。在用药前、用药后2周分别测定两组患者血N-末端脑钠肽、左室射血分数、左室舒张末内径、重新评价NYHA分级,肝、肾功能的变化。结果:治疗组显效率和总有效率均显著高于对照组。两组内治疗后2周左室射血分数、N-末端脑钠肽浓度、NYHA分级均较前明显改善,组间比较差异有统计学意义,两组治疗前后肝肾功能变化比较,差异无统计学意义。结论:益气活血利水方可以显著改善扩张型心肌病严重心衰患者的心功能及临床症状,值得临床推广应用。

Objective： To observe effcacy and safety of the Yiqi Huoxue Lishui decoction on severe heart failure of dilated cardiomyopathy. Methods： 45 cases were randomly divided into the treatment group （23 cases） and control group （22 cases）, who suffering dilated cardiomyopathy, with III-IV of NYHA class. The control group was given standard anti heart failure treatment; the treatment group were treated with the Yiqi Huoxue Lishui decoction more, for 2 weeks. NT-ProBNP, LVEF, LVEDd, NYHA and liver and renal function were measured before treatment and in 2 weeks. Results： The signiifcant effciency and total effciency in the treatment group were signiifcantly higher. In 2 weeks, LVEF, NT-ProBNP and NYHA were signiifcantly improved; and the difference between two groups has statistical signiifcance. The changes of liver and renal function before and after treatment showed no statistical signiifcance. Conclusion： The Yiqi Huoxue Lishui decoction can signiifcantly improve cardiac function and clinical symptoms, and is worthy of clinical application.
