灾后重建对德阳院前急救效率的影响

Effect of post-disaster reconstruction on first-aid efficiciency of Deyang city

目的通过比较2008年汶川地震重建前后德阳地区院前急救情况,分析灾后重建对德阳市院前急救效率的影响。方法通过发放调查表和查看120出诊记录等形式,随机收集了德阳市2007年度和2011年度"120"网络医院的出诊记录资料;2007年度为对照组,灾害救援后的2011年度为研究组。比较两组病例数量、疾病构成比方面的差异,以及两组在出诊时间、到达现场时间、返回时间和急救时间等指标方面的差异。结果经过灾后重建,德阳市的院前急救资源配置得到了优化和加强;灾害救援前后相比,院前急救病例基础的人口学资料并无统计学差异(P>0.05);而在120调度时间、到达时间和返回时间等方面,研究组较对照组明显缩短(P<0.05),分别为(1.79±0.63)min vs(2.36±0.52)min,(13.81±5.23)min vs(15.13±6.34)min,(13.25±5.18)min vs(14.02±4.66)min;死亡率显著降低(P<0.05),6.82%vs9.03%)。结论灾后重建促进了院前急救效率的提高,也是解决制约德阳院前急救医疗保障体系发展的有效方法。

Objective To analyze the effect of post-disaster reconstruction on first-aid efficiency of Deyang city by comparing the first-aid condition before and after ＂5 ·12＂Wenchuan earthquake rescue. Methods Prehospital emergency visit data of ＂120＂emergency net medical hospitals in Deyang city in 2007 and 2011 were collected by the questionnaire and the emergency visit records and the emergency resources changes between 2007 and2011 were compared. The data in 2007 and 2011 was designed as the control group and the study group,respectively. The differences of cases number,disease constituent ratio,visit time,arrival time,return time,and rescue time of pre-hospital medical care were compared between the two groups. Results The Deyang first-aid resources were significantly optimized and improved after post-disaster reconstruction（ P〉0. 05）. The demographic data had no significant difference between the two groups（ P〉0. 05）. The dispatch time,arrival time and return time in the study group were（ 1. 79 ± 0. 63） min,（ 13. 81 ± 5. 23） min,（ 13. 25 ± 5. 18） min,respectively,which were signifncantly shorter than those in the control group,which were（ 2. 36 ± 0. 52） min,（ 15. 13 ± 6. 34） min,（ 14. 02± 4. 66） min,respectively. The pre-hospital death rate in the study group（ 6. 82%） was lower than that of the control group（ 9. 03%,P〈0. 05）. Conclusion The post-disaster reconstruction has significantly improved the efficiency of pre-hospital medical care. To attach great importance to improve the rural pre-hospital medical care will probably be the way to solve the development problem of pre-hospital medical care in Deyang city.
