中重度阻塞性睡眠呼吸暂停综合征患者持续气道正压通气治疗前后糖化血红蛋白水平的变化 被引量:7

摘要 目的观察中重度阻塞性睡眠呼吸暂停综合征(OSAS)患者糖化血红蛋白(HbA1c)水平的变化,评估持续气道正压通气(CPAP)治疗对中重度OSAS患者HbA1c水平的影响。方法连续纳入2013年1月至2015年10月诊治的中重度OSAS患者330例。根据HbA1c水平将患者分为:〈6.0%(A组,n=116)、6.0%~6.5%(B组,n=110)和≥6.5%(C组,n=104)3组。所有患者行CPAP治疗3个月。治疗前后均行过夜多导睡眠监测和HbA1c检测。结果与A组比较,B组和C组的空腹血糖水平较高(P〈0.05),但B组与C组间的差异无统计学意义(P〉0.05)。CPAP治疗3个月后,C组HbA1c水平和血氧饱和度(SpO2)≤90%时间占总睡眠时间百分比(T(SpO2)〈90%)明显低于治疗前(P〈0.05)。Pearson相关分析显示,年龄(r=0.44)、收缩压(r=0.57)、舒张压(r=0.50)、空腹血糖(r=0.64)、呼吸暂停低通气指数(AHI)(r=0.68)、平均SpO2(r=-0.53)、最低SpO2(r=-0.28)、T(SpO2)〈90%(r=0.35)与HbA1c水平相关(均P〈0.05)。多元线性回归分析显示,平均SpO2(β=-0.907)和AHI(β=0.460)是HbA1c的影响因素(均P〈0.05)。结论中重度OSAS患者HbA1c水平与低氧血症严重程度相关。CPAP治疗3个月后,HbA1c≥6.5%的中重度OSAS患者有所改善。
