妊娠期糖尿病合并亚临床甲减对妊娠结局的影响研究

Effects of gestational diabetes mellitus complicated with subclinical hypothyroidism on pregnancy outcome

目的观察和研究妊娠期糖尿病（GDM）合并亚临床甲状腺功能减退（SCH）对妊娠结局的影响。方法选取200例GDM孕妇作为GDM组,200例SCH孕妇作为SCH组,100例GDM合并SCH孕妇作为GDM合并SCH组,200例正常妊娠孕妇作为对照组。对4组孕妇的妊娠并发症及新生儿的不良妊娠结局的发生率进行观察和比较。结果 GDM组孕妇羊水过多、产后出血的发生率高于其他3组（χ2=4.127～13.328,P〈0.05）,GDM合并SCH组孕妇妊娠期高血压疾病、妊娠期蛋白尿、剖宫产的发生率高于其他3组（χ～2=11.812～22.414,P〈0.05）;GDM合并SCH组新生儿早产、低出生体质量发生率高于其他3组（χ2=5.838～14.747,P〈0.05）。结论 GDM合并SCH可导致孕妇及其子代不良妊娠结局的发生率上升。

Objective To observe and study the effects of gestational diabetes mellitus（ GDM） complicated with subclinical hypothyroidism（ SCH） on pregnancy outcome. Methods A total of 200 pregnant women with GDM were selected as the GDM group,200 pregnant women with SCH were selected as the SCH group,100 pregnant women with GDM complicated with SCH were selected as the GDM with SCH group,and 200 normal pregnant women were selected as the control group. The incidence rates of pregnancy complications and adverse pregnancy outcomes in the four groups were observed and compared. Results The incidence rates of amniotic fluid increasing and postpartum hemorrhage in the GDM group were significantly higher than those in the other three groups（ χ2= 4. 127 ～ 13. 328,P〈0. 05）,and the incidence rates of gestational hypertension,gestational proteinuria,cesarean section in the GDM with SCH group were significantly higher than those in the other three groups（ χ～2= 11. 812 ～ 22. 414,P〈0. 05）. The incidence rates of premature birth,low birth weight in the GDM with SCH group were significantly higher than those in the other three groups（ χ2= 5. 838 ～ 14. 747,P〈0. 05）. Conclusion GDM complicated with SCH can lead to the increases of the incidence rate of adverse pregnancy outcomes in pregnant women and their offspring.
