摘要
目的 探讨妊娠糖尿病（GDM）与CDKN2A/2B基因多态性的相关性及其危险因素。方法 以2015年1月~2015年12月就诊于我院妇产科的孕妇为研究对象,纳入GDM孕妇（GDM组）65例,同期糖耐量正常孕妇（NGT组）65例。检测两组孕妇CDKN2A/2B基因rs10811661位点多态性,并对可能影响GDM的因素进行分析。结果 CDKN2A/2B基因rs10811661位点表现为CC/TC/TT三种基因型,CC、TC和TT基因型所占比例,NGT组分别为26.15%、43.08%和30.77%,GDM组分别为6.15%、49.23%和44.62%,两组基因型分布差异具有统计学意义（P=0.007）。NGT组和GDM组C和T等位基因分布频率分别为47.69%、52.31%和30.77%和69.23%,两组差异具有统计学意义（P=0.005）。携带T等位基因（TC＋TT）的GDM发病率更高（P=0.002）。此外,携带T等位基因（TC＋TT）、年龄、体质量指数（BMI）、C反应蛋白、胰岛素抵抗指数（HOMA-IR）、胰岛β细胞功能指数（HOMA-β）,GDM组和NGT组比较差异具有统计学意义（P〈0.05）。结论 GDM发病风险与CDKN2A/2B基因rs10811661位点多态性相关,而携带T等位基因、年龄、BMI、C反应蛋白、HOMA-IR和HOMA-β可能是GDM发病的独立危险因素。
Objective To investigate the correlation on gestational diabetes mellitus （GDM） with CDKN2A/2B gene polymorphism and its risk factors. Methods A total of 130 cases of pregnant women in our hospital from January 2015 to December 2015 were selected as research objects. Among them 65 cases of GDM （GDM group） and 65 cases of normal pregnant women （NGT group） were recruited. The CDKN2A/2B rs10811661 gene polymorphism were detected by PCR- RFLP, and the factors related to GDM were analyzed. Results The genetype CC, TC and TT in GDM group and NGT group were 6.15%, 49.23%, 44.62%, and 26.15%, 43.08% and 30.77% respectively, which were statistically different between two groups （P=0.007）. The frequency of C and T allele in the GDM group and NGT group were 30.77%, 69.23% and 47.69%, 52.31%, respectively. There was statistically significant difference between two groups （P= 0.005）.In addition, carrying the T allele （TC＋TT）might increase the GDM risk （P=0.002）. Multivariate analysis showed that carry the T allele （TC ＋ TT）, age, BMI, C reactive protein, HOMA-IR and HOMA-β might be independent GDM risk factors （P〈 0.05）. Conclusion GDM may be associated with the polymorphism of CDKN2A/2B rs10811661 gene. The T allele, age, BMI, C reactive protein, HOMA-IR and HOMA-βmay be the independent risk factors of GDM.
