摘要
目的通过测算护理工作量,探讨神经外科各专业组间护理人力的合理配置。方法采用自我记录法测定124项护理项目工时;借助医院信息系统获取各项目发生频次;计算神经外科各专业组患者人均日护理时数和床护比。结果神经外科各专业组患者人均日基础护理时数、观察性护理时数、治疗性护理时数、健康教育时数、医护配合时数、间接护理时数及总护理时数不同,差异有统计学意义(均P<0.05)。颅脑损伤专业组需要配置护士人数最多,其次为颅底脑干肿瘤专业组,而颅内外沟通性病变和幕下肿瘤专业组需要配置护士人数较低。结论神经外科各专业组间护理工作量不同,所需配置床护比各异,应根据疾病特点优化神经外科护理人力配置。
Objective To explore how to allocate the nursing manpower resources reasonably through calculating nursing workload in neurosurgical department.Methods The time it took to conduct 124 nursing tasks was recorded and the average frequencies of each task per day were calculated using Hospital Information System.Then,the bed-to-nurse ratio and the daily care hour one patient needed in different wards were calculated.Results Significant differences were found between the average daily basic nursing hour,observative hour,therapeutic hour,health education hour,doctor-nurse cooperation hour,indirect nursing time,and total nursing time among different wards of the neurosurgical department(P〈0.05 for all).The most nursing staff were needed in craniocerebral injury ward,followed by skull base and brain stem cancer ward,while the least were needed in the intracranial and extracranial diseases and subtentorial cancer wards.Conclusion Nursing workload and theoretical bed-to-nurse ratio were different in different wards of the neurosurgical department.Nursing human resources should be allocated according to the characteristics of diseases.
