光动力疗法联合玻璃体腔内注射抗VEGF药物治疗老年黄斑变性的临床效果

Clinical effect of photodynamic therapy combined with intravitreal injec-tion of anti-VEGF drugs in treatment of age-related macular degenera-tion

目的探讨光动力疗法联合玻璃体腔内注射抗VEGF药物治疗老年黄斑变性的临床效果,为其临床应用提供理论依据。方法选取2013年1月~2015年9月我院收治的120例老年黄斑变性患者作为研究对象,随机分成观察组和对照组,各60例,对照组采用玻璃体腔内注射抗VEGF药物进行治疗,观察组在对照组的基础上采用光动力疗法进行联合治疗。分析两组患者的视力恢复情况、治疗前后最佳矫正视力变化情况及术后并发症情况。结果观察组总有效率为95.00%,对照组73.33%,差异有统计学意义（P〈0.05）;观察组视力恢复要好于对照组,第1周和第12周最明显,两组差异有统计学意义（P〈0.05）;观察组总并发症发生率为5.00%,对照组13.33%,差异有统计学意义（P〈0.05）。结论光动力疗法与玻璃体腔内注射抗VEGF药物联合治疗老年黄斑变性,疗效显著,视力恢复能力强,最佳矫正视力最好,术后并发症少,值得临床推广。

Objective To discuss the clinical effect of photodynamic therapy（PDT） combined with intravitreal injection of anti-VEGF drugs in treatment of age-related macular degeneration（ARMD） and to provide theoretical basis for its clinical application.Methods 120 ARMD patients treated in our hospital from January 2013 to September 2015 were selected and randomly divided into observation group（n=60） and control group（n=60）.Patients in control group were given intravitreal injection of anti-VEGF drugs,while patients in observation group were treated by anti-VEGF drugs combined with PDT.The vision recovery,changes of best corrected visual acuity（BCVA） before and after treatment,and postoperative complications in two groups were analyzed.Results The overall effective rate was 95.00% in observation group and 73.33% in control group,which was significantly different between two groups（P〈0.05）.The vision recovery was more significant in observation group than that in control group,which was most evident in week 1 and week 12（P〈0.05）.The overall incidence rate of complications was 5.00% in observation group and 13.33% in control group,which was significantly lower in observation group（P〈0.05）.Conclusion PDT combined with intravitreal injection of anti-VEGF drugs has significant effect on ARMD,with favorable vision recovery,best BCVA,and few postoperative complications,which is worthy to be promoted.
