摘要
目的通过检测血清中mindin的水平,分析血清mindin与糖尿病肾损伤的关系。方法选取2型糖尿病(T2DM)住院患者96例,根据24小时尿微量白蛋白(urinary albumin,UALB)分为正常白蛋白尿组(A组,24 h UALB<30 mg)48例,微量白蛋白尿组(B组,30 mg≤24 h UALB<300 mg)25例,大量白蛋白尿组(C组,24 h UALB≥300 mg)23例,正常对照组(N组)20例为体检中心健康人群。检测糖化血红蛋白、肌酐、血清胱抑素C、24 h UALB;酶联免疫吸附试验(ELISA)测定血清mindin水平。所有统计分析结果均使用SPSS 17.0统计学分析软件进行。结果 (1)N组血清mindin水平显著低于A组、B组、C组(P<0.05);C组血清mindin水平明显高于A组和B组,A组、B组、C组、N组分别为(162.87±17.29)pg/m L、(347.19±16.01)pg/m L、(453.65±20.86)pg/m L、(89.87±10.20)pg/m L,差异均有统计学意义(P<0.05)。(2)Pearson相关分析示,血清mindin水平与24 h尿微量白蛋白、血清胱抑素C、尿素氮、肌酐等呈正相关(r=0.788、0.887、0.715、0.512,P<0.05);与糖化血红蛋白无相关性(r=0.112,P>0.05)。结论血清mindin可能反映糖尿病早期肾损伤,并可能与肾功能损伤程度相关。
Objective To evaluate the significance of serum mindin in the diagnosis of early diabetic nephropathy. Methods 96 patients with type 2 diabetes mullitus were devided to A group (n=48, normoalbuminuria, urinary albumin〈30mg/24h), B group (n=25, microalbuminuria, 30mg ≤ 24h urinary albumin〈300mg), C group (n=23, macroalbuminuria, urinary albumin≥ 300mg/24h), and N group (n=20,normal control group) were enrolled. The glycated hemoglobin, creatinine, cystainC, urinary albumin were measured by conventional methods. The levels of serum mindin were measured by using ELISA kits. The statisical analysis were made with the SPSS17.0 software. Results The level of serum mindin in the healthy physical individuals were significantly lower than in the patients of all 3 groups. Serum mindin in the patients of C group were significantly higher than in other 2 group. The level of serum mindin in petients with normoalbuminuria, microalbuminuria, macroalbuminuria and normal controls were (162.87±17.29) pg/mL, (347.19±16.01) pg/mL, (453.65±20.86) pg/mL, (89.87±10.2) pg/mL. The level of serum mindin were positively correlated with 24 h-urinary albumin (r=0.788, P〈0.05), cystatin C (r=0.887, P〈0.05), Urea nitrogen (r=0.715, P〈0.05), creatinine (r=0.512, P〈0.05). The level of serum mindin were no correlated with HbAlc (r=0.112, P〉0.05), Conclusion This study indicates that serum mindin might be closely ralated to diabetic nephropathy and participate in the progress of diabetic nephropathy. Serum mindin may probably reflect the early course of renal injury and be associated with renal function.
