补肾活血汤对激素性股骨头缺血坏死生物力学的影响 被引量:6

Study on Effect of Bushenhuoxue Decoction on Biomechanics of Steroid- induced Avascular Necrosis of Femoral Head

摘要 目的探讨补肾活血汤对激素性股骨头缺血坏死股骨头生物力学的影响。方法用贺氏造模法造模成功后,造模组随机分为模型组及治疗组。治疗组用补肾活血汤高剂量干预,空白组及模型组用等量生理盐水灌胃,三组在干预后6周和8周分别处死6只,取右侧股骨头做生物力学测定。比较三组间及6周和8周时,股骨头最大载荷和应变能力的变化。结果 1HE染色见:空白组:骨小梁完整,空骨陷窝较少,脂肪细胞较少。模型组:软骨细胞减少、软骨缺损;骨髓腔内填充大量的脂肪细胞,脂肪细胞增大或互相融合成大泡;部分骨细胞固缩或消失、骨陷窝空虚;骨小梁稀疏,部分骨小梁出现坏死。2 6周和8周时,股骨头应变能力和最大载荷值比较:空白组>治疗组>模型组(最大载荷值6周时是治疗组>空白组>模型组)。结论补肾活血汤增加了骨强度,改善了结构力学性能,促进了坏死骨组织的修复,可用于防治激素性股骨头缺血坏死。 Objective To evaluate the effect of Bushenhuoxue Decoction on biomechanics of steroid- induced avascular necrosis of femoral head( SANFH). Methods 104 New Zealand white rabbits were randomly divided into normal group and model group,once successfully modeling which was confirmed by HE staining of the femoral head,rabbits in model group were randomly divided into control group and treatment group. Rabbits in treatment group were administrated for high dose of Bushenhuoxue Decoction,while equal volume of saline was given in normal group and control group. After 6 weeks and 8 weeks,6 rabbits in each group were sacrificed,respectively. The right femoral head of each rabbit was taken out and used to study the change in biomechanics such as elasticity modulus and strain capacity. Results 1HE staining was used to evaluate the morphological changes.The femoral heads in normal group displayed rich bone matrix with normal chondrocyte number,the bone trabecula were in normal shape with structure continuity,few empty lacunae could be seen. While in model group,significant reduction of bone matrix and chondrocyte occurred or even disappeared,the tidal lines were discontinuous or even torn,large areas underwent necrosis,the bone trabecula became slender,sparse or disappeared,the marrow was filled up with plenty of adipocyte,fused with large vacuoles. Promising,morphological condition was greatly improved in treatment group,with more chondrocyte and richer bone matrix.Besides,the bone trabecula were more continuous,necrosis decreased,and adipocyte were decreased in marrow. 2After treatment,the Elastic Modulus( EM),strain capacity and maximum loading in treatment group were all better than those in model group,but not as good as those in normal group. Conclusion Bushenhuoxue Decoction is beneficial for the repair of necrotic bone tissue,it can improve the biomechanical property and strength of bone,which can be used as a novel therapeutics in SANFH.
