两种促排卵方案在体外受精-胚胎移植中的对比研究

A comparative study on two ovarian stimulation protocols in IVF-ET

目的研究两种促排卵方案在体外受精(in vitro fertilization,IVF)治疗过程中的妊娠结局。方法 2013年6月至2015年6月在四川省人民医院生殖中心行IVF助孕的患者460例,根据控制性促排卵(controlled ovarian hyperstimulation,COH)方案分为改良超长方案(A组)和长方案(B组)各230例,比较两组的助孕结局。结果两组患者年龄、不孕年限、总窦卵泡数、移植日内膜厚度、获卵数、成熟率、受精率、优胚率、可移植胚胎数、临床妊娠率、生化妊娠率、流产率、宫外孕发生率比较差异无统计学意义(P>0.05);两组剩余胚胎行囊胚培养结局,A组的囊胚形成率显著高于B组,差异有统计学意义(P<0.05)。结论与标准长方案比较,改良超长方案提升了囊胚形成率,并获得了良好的妊娠结局。

Objective To compare the pregnancy outcome of two ovarian stimulation protocols in IVF-ET.Methods We ret-rospectively analyzed 460 patients undergoing IVF treatment between June 2013 and June 2015 in our hospital .According to COH proto-cols,we compared the outcome of ICSI between modified super-long protocol （ group A,n =230 ） and long protocol （ group B,n =230 ） .Results No significant differences in age of patients ,duration of infertility ,total number of antral follicles ,endometrial thickness on transfer day,number of retrieved oocytes,mature rate,fertilization rate,rate of good embryo,portable embryo number,clinical preg-nancy rate,biochemical pregnancy rate ,miscarriage rate and incidence of ectopic pregnancy were found between the two groups （ P〉0.05 ） .However ,the blastocyst formation rate in the group A was statistically higher than that in the group B （ P〈0.05 ） .Conclusion Compare to long protocol ,modified super-long protocol increased the blastocyst formation rate .At the same time ,the protocol achieves a better pregnancy outcome .
