腹腔镜和小切口胆囊切除术治疗结石性胆囊炎患者的临床研究(80例)

Clinical study of laparoscopic and small incision cholecystectomy in the treatment for 80 patients with calculus cholecystitis

目的探讨腹腔镜和小切口胆囊切除术治疗结石性胆囊炎的临床效果。方法选择2014年1月至2016年1月结石性胆囊炎患者80例,按照住院号尾数奇偶随机分分为腹腔镜组和小切口组,每组40例。小切口组行小切口手术;腹腔镜组行腹腔镜手术。对比观察两组患者手术耗时、出血、切口长度、肛门排气、离床活动、住院时间和术后并发症发生率。结果腹腔镜组术后并发症发生率低于小切口组(P<0.05)。腹腔镜组患者手术耗时、出血、切口长度、肛门排气、离床活动、住院时间跟小切口组相比略有优势,但差异不具有统计学意义(P>0.05)。结论腹腔镜和小切口胆囊切除术治疗结石性胆囊炎均有一定效果,创伤小、术后恢复快等,但腹腔镜手术并发症更少,在临床应用可根据患者病情和经济能力进行选择。

Objective To study the clinical effect of laparoscopic and small incision cholecystedomy for the treatment of patients with calcu- lous cholecystitis. Methods 80 cases with calculous cholecystitis were selected from January 2014 to January 2016, which were divided into the laparoscopic group （40 cases） and small incision group （40 cases） randomizely. The patients in small incision group were taken small incision surgery, the patients in laparoscopic group were taken laparoscopic surgery. Compared time - consuming, bleeding, incision length, anus exhaust, leave bed adivities, length of hospital stay and postoperative complications of the two groups respectively. Results The incidence of postoperative complications of laparoscopic group was lower than the small incision group （P 〈 0.05） . Group of laparoscopic surgery was little better than small incision group of time - consuming, bleeding, incision length, anus exhaust, away from the bed adivities, length of hospital stay respedively, （P 〉 0.05 ） . Conclusions Laparoscopic and small incision cholecystedomy in the treatment of calculous cholecystitis have certain effect, which bring slight injury was, rapid recovery, and so on. Furthermore, laparoscopic surgery complications were more less, which could be a alternative choose in clinical according to patients condition and economic ability.
