电凝与超声刀止血对腹腔镜卵巢囊肿剥除术患者卵巢储备功能的影响

Effect of Ovarian Reserve Function by Electrocoagulation and Ultrasound Scalpel Hemostasis in Patients with Laparoscopic Ovarian Cyst Removal Surgery

目的:探讨电凝与超声刀止血对腹腔镜卵巢囊肿剥除术患者卵巢储备功能的影响。方法:选取本院妇科单侧卵巢囊肿患者300例作为研究对象,采用随机数字表法将患者分为三组,每组各100例。A组采用单极电凝止血,B组采用双极电凝止血,C组采用超声刀止血,采用化学发光法测定雌二醇(estradiol,E2)、卵泡刺激素(Follicle-Stimulating Hormone,FSH)、促黄体生成素(Luteinizing hormone,LH)、孕酮(progesterone,P)与睾酮(testosterone,T)水平,超声监测卵巢窦状卵泡数目(AFC)、卵巢动脉血流收缩期峰值(PSV)。结果:手术前,三组患者E2、FSH、LH、P、T、AFC、PSV比较差异无统计学意义(P>0.05);手术后3个月,A、B组E2、P、T、AFC、PSV明显低于手术前,FSH、LH水平明显高于手术前,手术前后比较差异均有统计学意义(P<0.05),A、B组E2、P、T、AFC、PSV水平明显低于C组,FSH、LH水平明显高于C组,比较差异均有统计学意义(P<0.05)。三组卵巢储备功能降低发生率与卵巢储备功能衰退发生率比较,差异均无统计学意义(P>0.05)。结论:电凝止血可能降低腹腔镜卵巢囊肿剥除术患者卵巢储备功能,而超声刀止血对卵巢储备功能几乎无影响。

Objective：To study the effect of ovarian reserve function by electro coagulation and ultrasound scalpel hemostasis in patients with laparoscopic ovarian cyst removal surgery.Method：Atotal of 300 patients with unilateral ovarian cyst of gynaecology department were selected as research objects,using the random number table method,they were divided into three groups,100 cases in each group.A group was given unipolar electro coagulation hemostasis,B group was given bipolar electrocoagulation hemostasis,C group was given ultrasound scalpel hemostasis,estradiol（E2）,Follicle-Stimulating Hormone（FSH）,Luteinizing Hormone（LH）,progesterone（P）,testosterone（T） levels were detected by chemiluminescence determination,AFC and PSV were monitored by ultrasonic.Result：Before the operation,E2,FSH,LH,P,T,AFC,PSV among three groups compared,with no statistically significant comparative differences（P〉0.05）;3 months after the operation,E2,P,T,AFC,PSV of A,B group were obviously lower than that before the operation,FSH,LH levels were significantly higher than those before the operation,before and after surgery compared with statistically significant difference（P〈0.05）,E2,P,T,AFC,PSV of A,B group were lower than those in C group,FSH,LH levels were significantly higher than those in C group,the differences were statistically significant（P〈0.05）.Three groups of ovarian reserve function reducing incidence and the ovarian reserve function decline incidence rate compared with no statistically significant differences（P〉0.05）.Conclusion：Electro coagulation hemostasis can reduce ovarian reserve function in patients with laparoscopic ovarian cyst removal surgery,and ultrasound scalpel hemostasis almost has no effect on ovarian reserve function.
