社区食管癌病人延续照护方案研究

Exploration of continuing care program for community esophageal cancer patients

[目的]探索社区食管癌病人延续照护方案的建立。[方法]通过查阅相关文献,根据食管癌高发区医院及社区卫生服务中心关于食管癌的护理实践经验,充分考虑专家意见,与病人需要结合,设计延续照护方案。[结果]构建出适合豫北食管癌高发区的"医院-社区-家庭"三位一体的食管癌病人延续照护方案,确立了延续照护方案应用效果的评价指标。[结论]延续照护方案的建立对于实现社区食管癌病人的个性化护理,提高其生活质量具有重要意义。

Objective：To probe into the establishment of the continuing care program for patients with esophage-al cancer in community.Methods：Through conswlting the relevant literatures,according to the experience of nursing practice in high incidence of esophageal cancer hospital and community health service centers,give full consideration to the opinions of experts,and combing the patients requires to design a plan of continuing care. Results：To construct the continuing care program of esophageal cancer patients in“hospital community fami-ly”trinity in Yubei district with a high incidence of esophageal cancer,further established the evaluation index of application effect of continuing care plan .Conclusion：The establishment of continuing care program was of great significance for the realization of personalized nursing of patients with esophageal cancer in community and improving the quality of life.
