颞下幕上经天幕入路切除岩斜区肿瘤11例分析

Analysis of 11 cases with petroclival tumor resected via subtemporal supratentorial transtentorial approach

目的总结颞下幕上经天幕入路治疗岩斜区病变的方法及经验。方法回顾性分析颞下幕上经天幕入路治疗岩斜区肿瘤11例的临床资料。结果 11例患者中,肿瘤全切10例,次全切除1例。无死亡病例。术后症状完全消失6例,症状缓解5例,未出现严重的术后并发症。结论颞下幕上经天幕入路到达岩斜区病变路径较短,无需磨除岩骨骨质,可直视岩骨背侧病变,对于岩斜区肿瘤具有较大的暴露范围和角度。

Objective To summarize the methods and experiences of surgery for the petroclival tumor via subtemporal supratentorial transtentorial approach. Methods The data of 11 cases with petroclival tumor resected via subtemporal supratentorial transtentorial approach were retrospectively analyzed. Results Of 11 cases, total resection was achieved in 10 cases and subtotal resection in 1 case. There were no death cases. The preoperative symptoms disappeared in 6 cases and relieved in 5 cases. No serious postoperative complications were seen in all cases. Conclusion The subtemporal supratentorial transtentorial approach for the surgery of petroclival tumor is shorter with the advantages of no need for grinding the bone of petrous, directly exposing the petrous, and providing larger angle and range of the petroclival tumors.
