重症胰腺炎患者营养风险筛查与临床结局关系的研究

Research on relationship between nutrition risk screening and clinical outcome of severe acute pancreatitis patients

目的调查重症胰腺炎(SAP)患者营养风险状况,分析营养风险与SAP患者临床结局的相关性。方法对90例SAP患者进行营养风险筛查,依据NRS2002营养筛查分数,将90例SAP患者分为A组(<3分)和B组(≥3分),进行营养支持调查。观察2组住院期间患者不良结局及并发症的发生率,比较2组患者相关实验室指标。结果 90例患者中,59例患者存在营养不良风险,31例患者营养正常。A组的BMI、白蛋白、前白蛋白均显著低于B组(P<0.05),2组血红蛋白、红细胞计数和空腹血糖差异无统计学意义(P>0.05);A组患者监护时间及住院时间显著长于B组(P<0.05),且A组胰腺周围脓肿、胰瘘发生率显著高于B组(P<0.05)。结论 NRS2002在SAP患者营养状况评价中有较好的适用性,临床需要加强营养风险评估。

Objective To investigate the nutrition risk of severe acute pancreatitis( SAP) patients,and to analyze the relationship between nutrition risk and clinical outcome of pancreatitis.Methods According to NRS2002 nutrition screening score,90 SAP patients were randomly divided into the group A( score < 3) and B group( score≥3),both with nutrition support. The occurrence of adverse outcome and complications of patents during hospitalization were observed,and lab indexes of two groups were compared. Results Of 90 patients,59 cases with malnutrition and 31 cases with normal nutritional status. The body mass index,levels of albumin and prealbumin were lower in group A than those in group B( P < 0. 05). There was no significant difference in levels of hemoglobin,red blood cell count and fasting blood-glucose between two groups( P > 0. 05). The surveillance time and length of stay were significant longer in group A than those in group B( P < 0. 05),and the incidence of peripancreatic abscess and pancreatic fistula were significant higher than those in group B( P <0. 05). Conclusion NRS 2002 has a good adaptability in assessing nutrition situation of SAP Patients. It is highly requited to strengthen and regulate the clinical nutrition risk screening path.
