疑难病例析评第370例动静脉栓塞-肾损害--嗜酸粒细胞增多--抗磷脂抗体阳性

摘要 病历摘要 患者男，25岁，因“反复咳嗽、咯血1个月余，胸痛、气促8d”于2014年7月6日入院。患者于2014年6月初无明显诱因出现阵发性咳嗽伴痰中带血，于当地医院行CT肺动脉造影（CTPA）示左肺下叶肺动脉栓塞，双肺下叶炎症、双侧胸膜炎。予低分子肝素、华法林抗凝治疗及抗感染治疗，监测国际标准化比值（INR）在2．00～4．88。6月28日开始出现胸痛、气促伴低热，复查CTPA示左肺下叶肺动脉内充盈缺损仍可见，为进一步诊治来我院。
