髓芯减压后自体MCS移植治疗早期股骨头坏死效果 被引量:6

CORE DECOMPRESSION AND AUTOGRAFT OF MESENCHYMAL STEM CELLS FOR EARLY STAGE AVASCULAR NECROSIS OF FEMORAL HEAD

摘要 目的探讨髓芯减压后自体骨髓基质干细胞(MCS)移植治疗早期股骨头坏死的效果。方法将104例早期股骨头坏死病人随机分为研究组和对照组,每组52例。研究组病人给予髓芯减压后自体MCS移植,对照组病人给予单纯髓芯减压治疗。比较两组病人治疗后1年的Harris评分、MRI低信号区域面积、疼痛缓解情况、6 min步行距离和术后再行人工全髋关节置换术(THA)率。结果术后1年,研究组病人各指标与对照组比较,差异有显著性(t=2.216~4.925,χ~2=4.875,P〈0.05)。结论髓芯减压后自体MCS移植,可以显著改善早期股骨头坏死病人的临床症状并改善其生活质量,临床上值得推广应用。 Objective To evaluate the clinical efficacy of core decompression and autograft of mesenchymal stem cells( MCS)for early stage avascular necrosis of femoral head( ANFH). Methods A total of 104 patients with early stage ANFH were evenly randomized to study group and control group. Patients in the control group were treated with core decompression and those in the study group were treated with core decompression and autograft of MCS. One year after the therapy,necrosis area,the Harris score,area of low MRI,easement of pain,6-minute walking distance,and the rate of total hip replacement( THR) were compared between the two groups. Results One year after surgery,the differences in the above mentioned parameters between the two groups were significant( t = 2. 216- 4. 925,χ~2=4. 875,P 〈 0. 05). Conclusion Core decompression and autograft of mesenchymal stem cells can notably improve clinical symptoms and quality of life in patients with early stage avascular necrosis of femoral head,this technique is worthy of clinical popularization.
