经尿道双极等离子前列腺剜除术与电切术术后尿失禁发生率比较

TRANSURETHRAL BIPOLAR PLASMAKINETIC ENUCLEATION OF THE PROSTATE VERSUS RESECTION OF THE PROSTATE:A COMPARISON OF POSTOPERATIVE URINARY INCONTINENCE

目的比较经尿道双极等离子前列腺剜除术(TPKEP)与经尿道双极等离子前列腺电切术(TPKRP)术后尿失禁发生率。方法选择100例良性前列腺增生病人,随机分为TPKEP组和TPKRP组各50例。以1h尿垫试验作为评判尿失禁的标准,观察比较两组拔除导尿管24h、2周以及术后1、3、6个月尿失禁情况。结果拔除导尿管24h时TPKEP组尿失禁发生率明显高于TPKRP组,差异有统计学意义(χ2=4.110,P<0.05);拔除导尿管2周及术后1、3个月时两组尿失禁发生率比较差异无统计学意义(P>0.05)。两组均未出现永久性尿失禁。结论 TPKEP术后短暂性尿失禁发生率较TPKRP术后高,但永久性尿失禁发生率并不增加。

Objective To compare the incidence of postoperative urinary incontinence（PUI）between transurethral bipolar plasmakinetic enucleation of the prostate（TBPEP）and transurethral bipolar plasmakinetic resection of the prostate（TBPRP）.Methods This study consisted of 100 patients with benign prostate hyperplasia（BPH）,who were equally randomized to TBPEP and TBPRP groups.One-hour urine pad test was adopted as the standard for evaluating urinary incontinence.The urinary incontinence-24 hours and 2weeks after removing the urethral catheter,as well as 1,3and 6months after surgery-in the two groups was observed and compared. Results At 24 hours after pulling out the catheter,the incidence of urinary incontinence in patients in TBPEP group was higher than that in those in TBPRP（χ2=4.110,P〈0.05）,after 2weeks of removing the catheter,and 1and 3months later,the difference in the incidence of urinary incontinence between the two groups was not significant（P〈0.05）.No permanent urinary incontinence was recorded in both groups. Conclusion The incidence of postoperative temporary urinary incontinence is higher in patients undergoing TBPEP versus TBPRP,but the incidence of permanent urinary incontinence does not increase.
