老年慢性主观性耳鸣严重程度与睡眠质量的相关性研究

Analysis of Sleep Quality in Elderly Patients with Chronic Subjective Tinnitus

目的分析老年慢性主观性耳鸣患者的睡眠质量情况。方法纳入2015-01/2015-12月,作者医院门诊就诊的老年慢性主观性耳鸣患者120例,以耳鸣致残量表（tinnitus handicap inventory,THI）评估耳鸣严重程度,以匹兹堡睡眠质量指数（Pittsburgh sleep quality index,PSQI）调查睡眠质量,对比不同严重程度耳鸣患者的睡眠质量评分情况和发生睡眠异常的比例。结果本组轻度（18≤THI≤36分）、中度（38≤THI≤56分）和重度（THI≥58分）耳鸣患者分别约占42.5%（51/120）、33.3%（40/120）和24.2%（29/120）,重度耳鸣患者的PSQI评分明显高于中度和轻度耳鸣患者（9.8±5.1 vs 7.5±4.3 vs 6.5±3.2,F=6.035,P〈0.05）,重度耳鸣患者发生睡眠异常的比例明显高于中度和轻度患者（72.4%vs 47.5%vs 27.5%,χ~2=15.253,P〈0.05）,患者THI评分与PSQI评分存在显著正相关（r=0.402,P〈0.05）。结论老年慢性主观性耳鸣患者的严重程度与睡眠质量存在相关性,耳鸣越严重发生睡眠异常的风险越高,应加强对患者睡眠质量的关注和干预。

Objective To analyze the relationship of sleep quality with chronic subjective tinnitus（CST） for elderly patients. Methods From January 2015 to December 2015, 120 elderly outpatient with CST in authors＇ hospital were included into the study. The severity degree of tinnitus and the sleep quality of the elderly patients was measured by tinnitus handicap inventory （THI）, Pittsburgh sleep quality index （PSQI） respectively. The sleep quality score and the sleep dis- turbance percentage of different severity tinnitus patients were compared. Results The mild （18≤THI≤36） , moderate （38≤THI≤56） and severe （THI≥58） tinnitus approximately accounted for 42. 5% （51/120）, 33.3% （40 / 120） and 24. 2% （29 / 120） respectively. The PSQI score of the severe tinnitus patients was significantly higher than that of moderate and mild tinnitus patients （9. 8 ± 5. 1 vs 7. 5 ±4. 3 vs 6. 5 ± 3. 2, F= 6. 035, P〈0. 05）. The sleep disturbance percentage of the severe tinnitus patients was significantly higher than that of moderate and mild patients （72. 4% vs 47. 5 % vs 27. 5 %,X2 =] 5. 253, P〈0. 05）. The THI score and PSQI score were significantly positive correlation （r = 0. 402, P〈 0. 05）. Conclusion There is a positive correlation between the severity tinnitus and the sleep quality in elderly patients with CST. The more serious the tinnitus is, the higher the risk of sleep disorders will be. It should be pay more attention and intervention on the sleep quality of the elderly patients with CST.
