摘要
目的比较实时PCR法与细菌培养法对妊娠晚期孕妇B族溶血性链球菌(GBS)带菌状态检测的灵敏度和特异度。方法收集本院2015年妊娠35周~39周产前检查的350例孕妇的直肠、阴道拭子进行2种方法 GBS检测。结果总体GBS阳性率为8.0%。细菌培养法阳性率(1.7%)明显低于实时PCR法(7.4%),差异有统计学意义(χ^2=11.82,P〈0.01)。若以两者均阳性为真阳性,细菌培养法和实时PCR法检测的灵敏度分别为21.4%和92.9%,两者差异有统计学意义(χ^2=26.32,P〈0.01);细菌培养法和实时PCR法检测的特异度分别为93.6%和99.4%,两者差异无统计学意义(χ^2=0.24,P=0.63)。实时PCR法的TAT时间为(10.3±6.1)h,72%能在当天出结果,28%能在第2 d出结果,而细菌培养法TAT时间为(58.2±12.5)h,两者差异有统计学意义(t=64.42,P〈0.01)。结论实时PCR法可提高晚期孕妇GBS携带者的检出率,对指导抗生素合理使用和降低新生儿死亡率有重要意义。
Objective To evaluate the sensitivity and specificity of real-time PCR assay and standard culture in the detection of group B Streptococcus( GBS) in women of pregnancy trimester. Methods Recto-vaginal swabs collected from women during the pregnancy of 35 w-39 w for prenatal examination were conducted for GBS test using the two above methods. Results Overall GBS positive rate was 8. 0%. The positive rate of bacterial culture( 1. 7%) was significantly lower than that of the real-time PCR( 7. 4%),and the difference was statistically significant( χ~2= 11. 82,P〈0. 01). If regarding two positive as true positive,the sensitivities of bacterial culture method and real-time PCR detection were 21. 4% and 92. 9%,respectively,with the differences statistically significant( χ^2= 26. 32,P〈0. 01),the specificity of bacterial culture and real-time PCR was93. 6% and 99. 4%,respectively,and the difference was not statistically significant( χ^2= 0. 24,P = 0. 63). The TAT time of real-time PCR time was( 10. 3 ± 6. 1) h,72% can get results in the day,28% can get results in the nest day,while,the TAT time of the bacteria culture method was( 58. 2 ± 12. 5) h,and the difference between the two methods was statistically significant( t = 64. 42,P〈0. 01). Conclusion Real-time PCR assay may improve the detection rate of GBS carriers in pregnant women. It is of great significance to guide the rational use of antibiotics and reduce the neonatal mortality.
